Observed Excessive Sweating. Smoke And Drink. Taking No Medication. Suggestions

Question: I am 34 years old male working with a leading MNC.

Since 2004, i have observed that i sweat a lot suddenly out of nowhere. My whole forehead, and backside of the scalp starts dripping. This i have observed specially
> when i am in a restaurant or in a mall, mostly in the public place
> when my seniors take a visit to my desk, many times (not always)
> when i XXXXXXX someone new or is introduced (sometimes)
> when i feel a place is hot or closed or small with many people inside stuffed

Other than this, i do not have any problem.
I am not on any medication, no abusive substance. I smoke about 10 cigarettes a day and drink once or twice in a month. i work hard a lot and in April and March, i was working about 15-16 hrs every day continuously. I enjoy making friends and dating.

My personal history to help you evaluate the cause:
In 2000, i had a break-up and i remember crying sometimes in the night before sleeping.
In 2001, i had an accident while driving although i was not hurt, it changed my risky & fast driving to an extent and created responsible driving sense.
In 2002, i did not have any job after completing studies in mid-2001 till mid Q1, 2003 due to 9/11. This gave me opportunity to do free-lancing by creating websites of friends, companies and interacting with them and earning some money.
In 2003, i got my first job in a company where i performed above average and worked late hours to explore solutions.
In 2004, i switched to my second job. The expectation were very high from my boss and assumed i know everything about application. From the third week of joining for next 1 year (till she went for her maternity leave), she used to humiliate me in public by making very low level statements, raising her voice that can be heard in the other corner, and extending my probation for 2 times back to back.
In 2005, my boss went on leaves for good and new boss came who built my confidence back by involving me and letting me interact with business, committing dates and guiding me and giving me more responsibilities.
In 2006, i was handling most of the application development, got promotion and till 2007 everything worked fine.
In 2008, my new boss promoted me / awarded me but soon my old boss came back and gave me new project (mid-size) with unrealistic dates and deadlines. I continued raising on XXXXXXX / calls with no use. As expected, it did not went well with half-baked application provided to users. All responsibilities came back to me and was working 24*7 for about 8 months with vendors and all stakeholders till it got stable.
In 2009, there was restructuring of the company and i was moved from service quality to new unit (O&T) and from that time onwards, all things started moving the way i dreamed of. My learning from past let me know, decide and take decisions accordingly.
In 2010, my girlfriend got married to her parent's choice after having 2 years relation due to caste differences which i was very upset and did not behaved the way i should have like forcing her to decide again, fighting on the call etc. A very childish behaviour! Same year, i found a new girl where we both got attracted and begin a new relationship.
In 2011, was promoted again and provided a challenging project.
In 2012, i got my dream role of managing and leading a big team in other country and doing it with great success acknowledged by all stakeholders. Personally, again my girls parents thought that i will not return back and even after convincing them that i will be back in year end, they forced her to marry.
In 2013, i was put into some other's project to take out from crisis and was once again appraised and now US & EMEA wants me in their respective projects. Personal relation is still better with my ex-girl and we talk over the phone every 2-3 days

It is good to see precise, nice and professional description of suffering with detailed elaboration of personal and occupational stressors.

In your query you mentioned about excessive perspiration and discomfort in different social situation. It is common symptom of symptoms of anxiety which may or may not be present in your case: nervousness, restlessness, mild irritability, excessive worry regarding own health, tremor, muscle spasm, heaviness in body, easy fatigability, medically unexplained pain complaints (headache, backache etc), sleep disturbance, palpitation and dry mouth.

Apart from that excessive perspiration can occur in thyroid hormone imbalance. Thyroid problem also increases chances of developing anxiety. It is better to get evaluated for serum T3/T4/TSH.

There is need for treatment, if symptoms cause significant personal distress or affect personal/professional life. Both drug and non drug treatment like cognitive behaviour therapy (form of psychotherapy) are used for treatment. Choice of treatment depend on severity of symptoms, personal preference and availability of treatment.

In drug treatment for long term relief most commonly SSRI (selective serotonin reuptake inhibitors) are used. SSRI takes around 3-4 weeks to show its effect. Non drug treatment like cognitive behaviour therapy (form of psychotherapy) also takes weeks to show its effect.

Apart from above advice following simple measures will help you:
-Decrease and stop smoking.
-Healthy diet: Milk and fruits in diet.
-Daily morning brisk walk
-Breathing exercise, yoga and meditation (which you can learn from some expert).

Thank you Doctor. In crisis, person do a lot of research online and becomes too confusing and your consultancy has really helped and very comforting to know finally the reason / cause. This is something what i was really expecting and I know everything has a solution.

My Thyroid (last checked in Q1 this year) is OK but have few other symptoms:
nervousness - only when sweating
easy fatigability - only after sweating
sleep disturbance - work timing cause it sometimes
muscle spasm - hand or leg (few times)
excessive worry regarding own health - yes
medically unexplained pain complaints (headache, backache etc) - common.
restlessness - no
mild irritability - no
tremor - no
heaviness in body - no
palpitation and dry mouth - no

I am here in India till 31st May and I will like to go for a drug based treatment here and not in the other country (language a major problem). Rather, i will prefer carrying medication for the prescribed time period.

I am not worried about side-effects much but should not be severe or threatening and if i can take general medicine like disprin / crocin , any fruit / vegetable / drinks / snacks that can conflict with the prescribed medicine.

I also understand that i will need prescription to buy it from the medicine shop like religare/apollo and also may have to show it if required/asked at airports.
