Question: My son is 13. He had a GI appt. this week but feeling worried. His biliruben is 3.1. It was 1.8 in 2018. his exam was normal. what could be wrong?

Hi dear,

From the details given to me, I won't be worried much.

Mild elevation of serum bilirubin in an otherwise normal child indicates a condition called Gilbert's syndrome. This is a totally benign condition, and need not be worried at all. Apart from the high bilirubin value, they can lead a normal life just like any other child.

But, to be sure, kindly give following details also:

1. Does he have any symptoms- like fever, jaundice(yellow sclera/eye balls), abdominal pain, itching or anorexia?
2. Any change in colour of urine(especially dark/yellow) or stools(especially grey or white)
3. Any significant illness in the past? Is he on any regular medication?
4. Finally, we need to get a split bilirubin testing(direct bilirubin and indirect bilirubin). Here, only total bilirubin is mentioned. Also, last part of report is missing in the image- kindly get a complete image

In short, as of now we need not worry; but get back with details so that we can be sure.

He has no past hx
of illness. No jaundice. His stomach hurt for a couple days last week and he was not eating well but this seems to have passed. This is not an ongoing issue. He normally is a healthy and active kid. Dr. Looked in eyes and said no jaundice. She said urine was normal but he needed to drink more. The last part of the report was a strep test that was negative. Dr. Also stated that upon feeling stomach, that is was normal and nothing felt enlarged. I don’t really have any other information at this time. Pediatrician has ran more blood work and we go to GI De. Later this week. Thanks. The waiting makes me anxious. Thanks for giving me some possible answers.

Thank you for the information

After going through above details, most probably this will be Gilbert's syndrome(provided this is persistent and it is the indirect fraction that is elevated). In this condition, child will be otherwise asymptomatic and fine. The last incidence of stomach hurting and poor appetite is probably unrelated to this, and may suggest a transient viral illness.

Other significant liver diseases are quite unlikely as he is otherwise asymptomatic, and liver enzymes(AST & ALT) are normal. Your gastroenterologist will discuss in more detail. Till then, there is no reason to worry and stay relaxed. I will be happy if you can inform me the final status.
