Question: My question is related to the digestive system, mainly the stomach and intestines. Im an occasional mdma user i take once every 4-5 months, but my digestive system has gotten worse after each use. I wanted to understand more why this happens, and how exactly do drugs like that affect the small intestines? its like they dont move and absorb what i consume as well as before. Is rifaximin for 2 weeks a good treatment for that? in all case id really love to understand the science of why this happens. Also many of my friends did not get these symptoms is it because my stomach is more sensitive?

Brief Answer:
Depletion of good Flora of intestine as a result of pyschoactove drug use

Detailed Answer:
Hello and thanks for choosing "Ask a Doctor" service for your query,
Have gone through your question in details my dear patient and can very well understand the questions going in your mind.

Firstly I would like to say that our gut especially intestinal parts consists of colonies of millions of different species of good bacteria's like Lactobacillus species and Yeasts like species of sachromyces just for an example. They help in proper digestion and complete fermentation of food we take.Any external substance like antibiotics or strong pyschoactive drugs etc cause severe depletion of these good Flora of our gut and gives a chance to bad bacteria and fungus to grow. Toxic bacteria and yeast ferments the food incompletely and releases large amount of toxins causing severe inflammation in intestines leading to poor food absorption pain etc. So when your intestine is inflammed it won't absorb food properly and you won't feel hungry. You can have a look on this article published by NCBI

https://www.ncbi.nlm.nih.gov/pmc/articles/PMC0000/

2)Not at all Rifaximin again is an antibiotic and would kill bad as well as good bacteria's of gut hence adding to your problems. Secondly it's a drug reserved for patient with irritable bowel syndrome.

3)As per your third query is concerned my dear patient the golden rule of medical science is that there is no rule. Every persons body is different and how it would response to a particular chemical is different. For e.g a person could memorise a book in 10 days while another takes a year to finish it. This is some thing determined by genetics immunity etc. So comparison could not be made between two different persons. Only recommendations could be given to you.

Suggestion-
Start taking a good over the counter probiotic twice daily like Econorm( Sachromyces) strain along with a probiotic containing Lactobacillus strain. This would help to replete your gut Flora and would increase the number of good bacteria's and yeast in your gut. These good bacterias would help to eliminate toxic colonies of bacteria's from your gut.
Take Yakult two times a day.
Add lots of salad to your diet.
One bowl of fresh veggies and fruits like pears apples kiwi should be taken once in a day.

Try light food for few days.
Get a stool culture test to rule out the presence of bacteria's in your intestines.
Fecal calprotectin to see level of inflammation in intestines.

For few days digestive supplements containing amylase lipase papain etc could be added to your diet. This would provide some rest to your inflammed intestines to recover.

Lastly try to not use pyschoactive drugs(Take it as your grand father's suggestion I am almost of his age only it would only cause problems in long runs).

Rest follow up with reports.
Till then start taking the over the counter products I suggested and try food modification.
Hope you would be fine very soon.
Do feel free to follow up,
Blessing and wish you a fast recovery and happy life.
May Lord bless you young man.
Any suggestions to make answer better are welcome.
Do rate the answer dear.

Thanks.

Thank you so much for all the info doctor! much appreciated :)
will get a stool culture and fecal calprotectin. are these 2 one and the same? will the stool culture indicate the amount of fecal calprotectin? or are they different tests? Also i got a probiotic called advancis biobaci probiotic - contains 4 kinds of lactobacillus and 3 kinds of bifidobacterium and saccharomyces (is this the same as sachromyses? ) dosage is 3 capsules a day each capsule has 2.5 billion is that too little? also should i take all 3 at once? or should i get 2 different probiotics that each contain ONLY lactobacillus and the other sachromyses) also when i have test results will send them to you

Detailed Answer:
Hello again,
Thank you for appreciation,
Answers are as follows,

1)Both of them require just a small sample of morning stool .However both are different the first one would show bacterial or protozoal growth while second one is an indicator of infection.

Keep on taking the probiotic.
Three times a day is good combination can even be extended to 4 times daily.

No take then at interval of 8 hours.

If possible add VSL#3 thrice daily to it.It contains high quantity of sachromyces and is available online.Your probiotic contains good amount of bacteria's but yeast is a bit low.

when we close the discussion i can rate you? if so will do so once we are done, and if theres a way plz tell me how hehe. Thank you doctor really for the advice! I did not tell you about my diet, i have been on a ketogenic diet for about 8 months, i started it when i was feeling the stomach problems, and at first it helped alot i improved soo much and have been working out also im very fit. After 8 months of keto im now transitioning into a healthy low carb diet with fruits and all kinds of healthy carbs like rice and quinoa etc but keeping the carb count a little bit low but not ketosis low anymore, only 3 days a week maybe (cycle keto). Also I do smoke medical marijuana before going to bed. My main issue i think is because of the mdma every few months, esch time it annoys me much more. so will definitely stop that and start this treatment! i think a healthy diet and good probiotics will do the trick. Should i also start with the extra overcounter supplements you suggested? Again thank you so much doctor

Brief Answer:
Yes, you should start my dear.

Detailed Answer:
Hello again,

As you were on a ketogenic diet you were probably taking a large amount of fat and a very small amount of carbohydrates especially complex carbohydrates. These complex carbohydrates contain large amounts of fibers which are very essential for proper digestion as well as the bulk formation of stool.

As per the researches, I have read all these fancy diets are usually designed for next-level athletes however if you have less then 8 to 10 years of experience these diets are given by dieticians just to justify the money they are charging.

PROTEIN is now a billion dollar industry as you can see by yourself. Most of the articles and researches done.

You can try even by yourself 9 out of 10 studies suggesting these fancy diets are sponsored by companies selling these products. On top of that just to remind you that during evolution species like Arcadians Spartans etc were much stronger then we and no one among them was following any specific diet.

See the first purpose of strength/resistance/Endurance training is to give us strength.
40:30:20 is medically approved ration that is 40% of your calories should come from complex carbs(Best source of fibers which promotes growth of natural gut flora and keep your stomach healthy the rest 20 percent from lean source of protein and 10% from red meat(Red muscle fibers contain a large number of blood vessels and hence they contain tonnes of myoglobin as well as vitamins especially all sorts of B complexes.

Rest 20%, should be fat in which saturated fat should be around 8% and unsaturated should be 12%.
Green vegetable and 3 colorful fruits should be a part of your diet daily.
All this information is taken from XXXXXXX journal of Cardiology as well as Bodybuilding and fitness encyclopedia by Arnold(Books) and other fitness freaks.

Along with this diet follow high intensity exercise for just 4 to 5 days a week.
A famous coach has said that if you are spending more then 60 minutes in the gym then you are just making friends there.

Don't fall prey of these fancy diet promoted by companies and designed by dieticians to justify their payments.

Follow what I said for 12 weeks and if you find that it's not helping you I assure you I would stop answering on this forum.
Diet and reading books by famous bodybuilders and researchers is my area of interest and I assure you with a properly balanced diet you would get much better results.
Definitely yes you should add a probiotic initially like VSL#3 twice daily till your gut flora replete itself along with it take natural Isapghul husk(fiber supplement available online on amazon) 10 gram 3 times a day.

Tell me how you feel after 2 weeks.

In case you are interested in high-intensity training or High volume training like topics you can revert me back. They are very large and I have read books written by schools of both these generas. However, if you are superfit I would suggest a high-intensity training rather then high volume(long workouts) training.

Sorry for a long answer.
Hope I was easy to understand.
Have a balanced diet. Though you can give try to a diet for a 3-4 week in between just to shock your body but not more than that.

Thank you.
You would be fine very soon.
Regards.
