Question: My girlfriend is experiencing pain at her fingertips.
There are sores developing there. Thought it was just dry skin splitting, but she has been using hand balm and other lotions for some time.
This has been going on for 3 weeks now.

There is redness just by the fingernail and skin has split in some spots. Extremely painful to touch.
Also has RA. Taking methotrexate for that.
Pramipexole for restless legs

The symptoms of your girlfriend seem to be related to rheumatoid arthritis.

I suggest her to use anti inflammatory medication such as Ibuprofen 400 mg three tablets a day.

I also recommend to use a moisturiser to apply frequently at the hands.

To relieve the pain she can also use warm compresses and do regular exercise of the fingers.
