Answered by Dr. Ketan Vagholkar (1 hour later)
Dear Mr. XXXXXXX
Thanks for the feedback
The first two symptoms which you have described in your feedback are due to acid reflux.
For this I would suggest the following
1. Reduce the volume of food intake at dinner.
2. Let there be a gap of at least 2-3 hours between dinner and bedtime.
3. Avoid alcoholic beverages/spicy foods/aerated waters.
4. Take 1 tsf of sucralfate at bed time to exert a protective effect on the lining of the upper gut..
5. You can also take a single dose of Cap omeprazole 20 mgm at bed time. this will reduce the acid output at night and enhance healing.
6. Ensure that your head end is at a higher elevation than the lower end while sleeping. this will reduce the pressure at the upper part of the gut.
For the time being just stick to these measures for at least 6 weeks. I am sure this will help you out.
For the constipation problem I would suggest plenty of water, XXXXXXX leafy vegetables and bananas. Banana is both a bulk former as well as a natural antacid and will give you good results. Avoid apples or any variant or product containing it as it would constipate your gut.It would not be ideal at your age to get habituated to isapgul though it is a very safe preparation.
The food additives advised by me are freely available in any metro city without prescription.
Fermented foods could cause increased flatulence which could increase your upper abdominal discomfort. Therefore try reducing them.
No apples or XXXXXXX products. They have constipating effect.
Vinegar taken occasionally is fine but not on a regular basis.
If you get relief by these measures the need for a H pylori test would not arise.
The carbon or urease breath test would be better than any sort of invasive biopsy culture.
I hope this answers all your querries.

I have been suffering from these problem last couple of days.
well it was there before also.
And I tend to have mucus problem at the slightest of food irregularities.

Any thought? what should I do for this?

Hi,
Thanks for the feedback.
The description of the symptom is typically suggestive of chronic duodenal ulcer.
The treatment for which is at least 6 week therapy of a combination of acid reducing agents along with coating agents.
If biopsies are normal you do not have to worry about the mucus problem.
I hope this answers your querry.

Hello Doctor ,
How r u ?

3 to 4 months back i used to take Colospa retard and pantocid for almost a month or more.
Then no medicine for sometime.

Now I have started taking (according to your suggestion)
Cap astymin forte 1after breakfast for 12 weeks.
Cap cobadex forte 1 after breakfast for 12 weeks.
cap mega 3 -1 after breakfast for 12 weeks.
tab neurotrat 1 after breakfast for 12 weeks.

1 tsf of sucralfate at bed time

a single dose of Cap omeprazole 20 mgm at bed time

Improvement:
Now I dont have the acidity , and the burning abdominal pain, usually in the area just beneath the ribs are improved now.

Offlate I have been having bland chicken items (last few days lot of food with jaggery ) , dont know if that is related.

Observations:

Upper abdominal discomfort surpass whenever I have some irregularities in food practice (like heavy food at night) , or have less water for long time but for short time.

And I have been farting a lot ,really a lot, with bad smell many times.
And passing 2 3 times a day , may be more , anytime in a day, but not clear.
(this is not new , but now little more )
And when I sleep or early morning much before leaving bed,I have back pain which always happens when my stomach is not clear (old problem of discrete back pain problem and it is always associated with irregular bowl movement for me). Some sleep disturbance because of that , morning doesn't feel so fresh , feel little sleepy or dizzy also.
And at night I sweat sometimes. What could be the causes for this?

Are there any side effects to those above mentioned medicines?
Do you want to suggest or add any thing else to that list for this Irregular bowl movement.
No improvement in weight , still on the decreasing slope, I would have to weight I guess.
And I am not doing any exercise offlate.
Will start cardio , some weight lift from next week.

Thats it.
Please reply with your valuable suggestion.

Hi,
thanks for writing in.
I happy to hear from you that you did get some relief.
You can continue with the medicationsI had advised. There is no chance ofany side effect whatsoever.
For the gas problem I can suggest one more medication.
SORBILINE one tablespoon with a glass of warm water in the morning after getting up . This can be taken for 12 weeks. It will regularise your bowel function.
Bowel problems always aggravate arthritic pains. I was worried to hear that you specifically experience backache when you have a bowel dysfunction. Have you done xrays of the lumbosacral spine and joints especially x XXXXXXX of the pelvis with both hips. Do you experience morning stiffness of the lower spine. If yes then in addition to x rays get a HLA B27 status evaluation of the blood done.(for ankylosing spondylitis)
I hope this answers your querry.

Hello Doctor,

Thanks for the reply.
I have never done the xray .

I have some erictile dysfunction problem which gets worse with the pain , which is obvious I guess.
Anyway I hv been on medication for that for some time.

But just read about Cauda Equina Syndrome which includes bowl dysfunction and sexual problem both.
SO i was just wondering if I need get some scan done.

Tomorrow I am getting the xray of what all you have mentioned done.
Do you think I need to go for scanning or something to make sure if atall I have any problem of that sort.

Any advice from your side will be really appreciated.

Thanks & regards XXXXXXX

Dear XXXXXXX
Thanks for the feedback.
It is unlikely to be cauda equina lesion. There is a high likelihood of vertebral arthritis which I had mentioned in the previous reply. Kindly do the following investigations.
1. X XXXXXXX lumbosacral spine (AP and lateral view)
2. X XXXXXXX pelvis with hips ( specifically for the sacroiliac joints)
3. HLA B 27 status of your blood sample.
As soon as the results of the above tests are available, you can write to me or even upload the x XXXXXXX films and reports.
Do not go for MRI immediately.
I hope this answers your querry.

Hello Doctor,
Sorry for the delay.
You asked me to get some tests done.
I will get the reports tomorrow and will send you the soft copies.

I am little worried now.
Will see any doctor in Manipal this weekend If I can.

The upper abdominal discomfort is less aftr using the medicines (OMEprazol and sucralfate ) , but it tends to hamper at the slightest of my food change.

Constipation also it is there and little improved with sorbolin , I mean I have to be on strict diet.

Sometimes I feel little dizzy my head spins a little.
I have been sleeping little less.
And night sweating is a problem sometimes.

The main problem is there is a constant decrease in my weight now.
Today I measured its 59.6.
2 weeks back when I 1st wrote you it was 60.6.Two months back i was 64.

Would you please suggest what all tests I need to do now ?
I want to know what all problem (less serious or serious) can lead to this constant weight loss.

The last endoscopy was done more than a year back.
And the colonoscopy was done 8 months back.

Thanks & regards XXXXXXX

Hi,
thanks for the feedback,
It is good to hear that you are feeling a bit better. Continue with medicines i had advised.
Regarding your weight loss problem. Since all your previous endoscopic investigations are normal I would suggest thyroid studies to be done to search for the possible cause of weight loss.
Kindly do T3,T4 and TSH levels done.
Write back to me after the reports are available.

Hello Doctor,

How are you?
You asked to get done some tests.

>>1. X XXXXXXX lumbosacral spine (AP and lateral view)

Result: Features are suggestive of paraspinal muscle spasm

>>2. X XXXXXXX pelvis with hips ( specifically for the sacroiliac joints)

Result : No significant abnormality seen

>>3. HLA B 27 status of your blood sample.

Result: Negative

>>T3,T4 and TSH levels

Result:

T3 81.9 [normal 58-159]
T4 6.27 [4.87 - 11.72]
THS 2.61 [0.5-5.0]

So I am little clueless about the weight loss .But i have thought not to let myself get much worried about it.As I am unable to do anything about it .

My motions are not clear at one or two go, and passing wind is always there.
If I take Isapgul sometimes, should I take it b4 food or after food.

I have some itching feeling in the rectum area many times in the day.

Back pain is more or less there sometimes.

Local physician suggested to take probiotic tablets sometimes.(Econorm) , which is pretty expensive .Please tell what do you think about taking those.

And last 2 weeks sometimes I feel little dizzy. And tired also at times. Can that happen because of less sleep or may b extra Gas?

Night sleep is always a concern , As I try to eat less in the dinner or atleast keep minimum 2 hrs of gap b/t sleep and dinner, sometimes b4 sleep only i feel little hungry.

I am thinking to start doing some exercises again.

Thanks & Regards XXXXXXX

Hello Doctor,
Thanks.
I will work on those.

The foods as you have suggested.
As a fresh fruit water melon or papya or guava is fine?

Should I avoid mashroom and Paneer?

I asked you about those probiotics, as you didnt mention anything , so I assume you are advising me not to have it right now.

And the Isapgul also.

Will get back to you later may be if anything urgent.
Thank you once again .

Hello Doctor,

Sorry to trouble you again.
3 days back I had peas , and started having stomach pain.
Last 3 to 4 nights morning after 4 am I am unable to sleep properly because of a burning sensation in the upper abdomen right at center bellow the rib joints.

This area has become little sensitive last few days as I experience some mild to pinching pain sometime in the day time also. But can be checked with light food as I am observing. But very morning early morning i dont get any relief .

Night after heavy food I experience stomach pain , in the upper abdomain.
Can you suggest any medicine?
I am continuing the medicines btw.

I am consuming curd at night, may be I have to stop this. What do you think?
This weekend I am planning to go to Kodai XXXXXXX a hilly area just for a change.
Do you think thats a good idea as it might hamper my regular food habit a little.

Thanks & Regards XXXXXXX

Hi,
Thanks for writing in.
From the description you have given it appears to be a excarbation of peptic ulcer disease. I would suggest the following
1. reduce the volume of food at dinner.
2. consume a bland diet at dinner.
3. avoid curds at dinner. You can take buttermilk instead at lunch.
4. sparacid 1 tsf three times aday after meals for 5 days.
5. cap omez 20mgms at bed time for 5 days.
You can travel if it is going to be relaxing. However do not change your diet significantly.
Kindly let me know the response.
