Is Treatment For Encephalomalatia Required Post Brain Injury?

Question: Hi Doctor, I was given a CT scan of the head in a hospital about 2 months ago after an episode of syncope due to essential hypertension, stress at work (extreme for >6wks., now over) and orthostatic hypotension. I was told to stop taking the Lorsartin 25 mg./day, which I had started just 1 week before, as my blood pressure upon discharge was 121/73, and 2 days later BP was still normal. I did not work or drive for the next 6 days. A cardiologist had me wear a heart monitor on me for 14 days, he said it was probably a vaso-vagal (sp?) reaction, but the monitor & stress test follow-up, which I have not yet received, are to rule out atrial fibrillation.
The CT of head said hypodensity in right basi frontal lobe likely due to prior injury, encephalomalacia. It has been 7 years since I last hit my head. Since resuming normal activity, I have had no symptoms, no memory issues, and feel normal. I also felt no symptoms or memory issues from the time 7 years earlier when I hit my head. Is there any need for treatment for the encephalomalacia?

I have read your question and understand your concerns.

Encephalomalatia is a common finding in imaging studies such brain CT scan and MRI, even years latter after significant traumatic brain injury.

Localization at frontal basal lobe may be related to stress disorder.

Encephalomalatia doesn't need any treatment itself, and can't be treated because it consists on brain cells damaged, and brain neurons are not reparable/regenerable, but, stress that may be related for example should be treated by medications and counseling.

So, you shouldn't worry about encephalomalatia, discuss with your Doctor about being treated for stress disorder.

In mean time, if there will be any condition found to be related to syncope ( arrhythmia, postural hypotension for example ), reevaluating the treatment is necessary.
