Is Thyroidectomy Necessary For Multinodular Goitre With Benign Cystic Degeneration?
Question: my father, 53 yrs, has got multinodular goitre. his fnac report is suggestive of the same with benign cystic degeneration. some doctors are saying thyroidectomy is necessary whereas some are saying its not. what shall we do.?

thyrox 50 micrograms.
Your Father has MNG- multinodular goiter, FNAC shows MNG with benign cystic degeneration.
Dilemma about whether to get operated or not:
I would like to know a few things before I can assist you more.
What is the size of the goiter? is it too large that it does not look good ?
Since when is the goiter present?
What was the report of thyroid function before thyrox 50 was started?
Why was thyrox started - was it really necessary or someone started for reducing the goiter that too MNG?
Any problem with swallowing or respiration?

On 25 XXXXXXX 2015, CT scan - neck revealed, right lobe of thyroid gland measures 6.6*5.1*4.8 cm. there is another small similar lesion on its inferior aspect measuring 2.4*1.6*1.4 cm. On observation at times it looks big and at times it reduces in size on its own.
On 5 march 2015, sonography of neck revealed right lobe of thyroid gland measuring 5.8*4*3 cm ans a smaller cyst within the gland parenchyme measuring 2.3*2.2 cm.
this is present since feb 2015.
befor starting thyrox 50mg, values were T3: 91 ng/dl, T4: 5.4 micro gm/dl and TSH: 4.0 IU/ml. this tablet was started in view of goitre.
at times he reports mild swallowing difficulty, however there is no problem with respiration.

Thyroid function looks normal, thyrox really does not help and on the contrary may cause other problems- rather physiological disturbance in the feedback mechanism on which the body works (although not a matter of concern at the moment)
There is also history of mild swallowing difficulty at times.
The size is too big.
Known diabetic under control 2 years.
I would go in favor of removal of the diseased part and send the specimen for histo-pathology.
