Is The HIV DUO 4th Generation Test Conclusive At 32 Days Post Exposure?

Question: hello doctor ,
I am a XXXXXXX and had unprotected oral on 1st of april and 19th of april
and got an HIV ANTIBODY , HIV PCR , HIV DUO , HIV P24 Ag test on 5 th of may all of them are non reactive , and not detectable on pcr , am i conclusively negative , i got a HIV DUO 4th generation test on 21 st may i.e ( 32 days post exposure ) are the results conclusive . i m having swollen lymph nodes slightly and slight head ache has began . after how many days of exposure ARS symptoms occur

Hi, welcome to HCM and thanks for posting your query. Usually, antibodies appear in the blood by 6-8 weeks or delayed up to 3 months. So HIV antibody would be conclusive after 3 months of exposure. HIV PCR, HIV DUO and HIV HIV p24 Antigen tests are conclusive in earlier period by 2-4 weeks but they are specific for HIV 1 only. As you had got all these done and all of them are non-reactive than I can say you are conclusively negative. As far as symptoms of ARS are concerned they will appear within 2-4 weeks after HIV infection and include fever (most common), swollen glands, sore throat, headache, muscle and joint pain, rashes and fatigue.

doctor my lymph nodes are swollen . could it be an indication of ARS , lymph nodes on chest , neck and thighs are palpable. but they would be less then 1 cm
. what is the risk of oral sex and transmission of HIV . because as mentioned above i had oral sex with a guy , i took his penis in my mouth . but he did not ejaculate in my mouth .

Can viral RNA PCR be conclusive at 15 days ???
I under went a HIV duo and p24 on 15 th may again result showed non reactive
and negative respectively .
I underwent a HIV duo on 21st may i.e 30 days post exposure result negative
what is worrying are my lymph nodes swollen .
The qualitative PCR i had has screened both HIV 1 diff sub types and HIV 2 gag gene.
DOCTOR please provide me with a solution what to do ??
as i m not able to sleep properly due to this .
What is the risk associated with my exposure .
I am planning to go for a retest on 31 st of may which would be six weeks of second exposure

The risk of HIV transmission through oral sex is much less than vaginal.

Dear XXXX, the risk of HIV transmission through oral sex is very less than vaginal or anal but it is not zero. HIV RNA PCR is conclusive in earlier period by 2-4 weeks. Your all the test results are negative or non-reactive so you need not to worry. You can go for repeat and I can say it will come negative also. As far as swollen lymph nodes are concerned it may be due to some other infection. You can consult your doctor for that and take treatment for the same. Thank you doctor , now which test at this point of time i should undergo to be conclusively negative its 34th day post exposure .
suggest me a test which will be conclusive at this point of time i.e 34 days after exposure.

You have to go for viral RNA PCR or HIV P24 antigen test at this point.

As antigen detection tests like viral RNA PCR or HIV P24 antigen are conclusive in earlier period of infection by 3-4 weeks so I will suggest you to go for the same tests at this point of time to be conclusive or confirmatory.
