Question: Hello, I am having Aortic valve replacement surgery for regurgitation along with ascending aortic aneurysm repair and I may be treated at the same time for minor possible afib on March 17th. I am a 57 yr old female, non smoker that has always been healthy other than being told since I was a small child that I had a heart murmur. My questions are how much of a risk is there for stroke and can an ascending aortic aneurysm repair using a graft cause paralysis and if so how much of a risk is there? I understand your concern and would like to explain that aortic valve replacement surgery with ascending aortic aneurysm repair (even using a graft) now has become a routine procedure and when performed in high-volume cardiac surgery centers it has a very high rate of success with quite a low potential for complications.

Now returning to your direct question, this kind of intervention (as other cardiac surgery procedures) has a potential for stroke.

In your case it will depend on different factors:

- The extend of aortic aneurysm through the aortic arch (possible involvement of cervical vascular branches)

- Also from structural changes of aorta walls (the presence of extensive atherosclerosis and calcifications may be a risk factor for embolic events or stroke)

-Another risk factor is the duration of extra-corporal circulatory support during surgery. The longer the circulatory assistance the greater the risk for neurological complications

-The last but not the least is the surgical team competence.

Despite the above mentioned potential underlying factors, the risk for stroke remains quite low, in general less than 3.3-5%.
