Radical prostatectomy if medically fit.

Your concern is understandable.
As an Urologist, having treated several people in the same situation, i must advise, that a radical prostatectomy, is almost curative.
If medically fit for surgery, that's the best option today. A robotic procedure is the ideal one. There's preservation of potency and continence with quick recovery.
Further hormonal therapy, will depend on repeat PSA.
