Brief Answer:
Post exposure prophylaxis HIV

Detailed Answer:
Hello XXXX
You asked a question "Does prophylaxis work if taken after 72 hours of exposure ?
Answer;
Yes, When Post Exposure Prophylaxis i.e. PEP initiated based on the result of a rapid HIV test , a confirmatory test should be performed , and if the test is NEGATIVE , PEP can be discontinued.
PEP should start at once whenever it available ( as in your case you started after 72 hours ) and it will work .
Viraday ( tenolovir Disoproxil fumarate 300 mg+Emtricitabine 200 mg+Etavirenz 600 mg)
For HIV prophylaxis basic two drug regime is preferred but in your case it is three drug regime , so this is at present most preferred PEP .

Just for your satisfaction I want to tell you one more thing :
Oral sex is a much less efficient mode of transmission of HIV ,because there is no convincing evidence that oral sex can transmit HIV infection either through kissing or other oral exposure including precum or semen.
So , need not to take any tension , you are not at risk for HIV .
Hope I have answered your question.
Further , if any question I will reply.
Good luck.

Thanks Doctor!

PEP was initiated because of my confirmed exposure to an HIV positive partner as a preventive measure. I had taken a test (Elisa) the next day and result was NON Reactive (Patients's count - 0.120). Should the count be 0? I understand that Elisa test needs to be taken after atleast 3 weeks of exposure. I will be taking another test after two weeks.

My partner is on HIV medications and he says his viral load in less than 50 (undetectable). This reduces the risk of transmission right? But I am not sure because I don't know him that well so I can't trust him.

Just after 3 days of medications (Viraday) i have been suffering from its side effects. A lot of dizziness and sleepiness. I can't work. Is it normal? My body is unusually warm and my whole body has slight dull ache. Are these symptoms for anything specific?

My doctor says since the risk is very low i can stop the PEP. But if i need to be very safe i an switch to Tenvir Em 1and Nevimmune 200 1 daily. Do they have the same side effects? Do i really need PEP in the first place or am I just overreacting?

Brief Answer:
HIV POSTEXPOSURE PROPHYLAXIS

Detailed Answer:
Hello XXXX.
I am replying question to question :
1 Elisa test result ( your partner is ( 0.120) .
Should be ZERO.
Ans :Normal value depend on the type of substance being identified. Normal value ranges may vary among different labs.
But ideally NEGATIVE ELISA test means No HIV .
Without proper substance and labs report very difficult to say accurately but if ELISA is negative means no HIV.
2 Your partner viral load is 50
is this reduce to transmission is right ?
Ans
Control of plasma viremia by effective anti retroviral therapy , particularly maintaining the VIRAL LOAD consistently LESS THAN 50 copies of RNA /mL.
So if viral load is <50 means under control
3 &4 :Three days viraday , side effects dizziness and sleepless .
Ans Yes, these are common side effects.
5 Are these side effects specific ?
Ans ; No these are common , not specific for you , so NEED NOT to worry.
Yes , your doctor is right , stop medicine .
Switch to Tenvir and neviiimune?
Ans
I don't recommend you to take these medicine .
Since I , mentioned in my previous reply that oral sex is much less efficient mode of transmission of HIV .
Saliva contains anti viral factors , so you can not get HIV , this is scientifically proved and written in standard books .
When , NO HIV , why medicine ?
In my opinion , XXXX, you are over reacting ,so don't take medicine , don't take tension.
Good luck for you.
