Question: had unprotected sex with gf after 2 days of her last period (unprotected sex on 31st jan) and then she took ipill in 12 hours. she is feeling headache, laziness and tired from 1st of feb. what are the chances of pregnancy and why is this happening to her?

The chances of her getting pregnant is negligible as you had intercourse during the safe period 2 days after menses stopped and as ovulation was yet to occur she was safe already.
However she took pill as well as double protection following which she may experience a withdrawal bleed in next 7-10 days but the bleed may be scary as pill was taken so early in menstrual month when endometrium has not yet formed completely.
The symptoms of laziness and headache are due to progesterone fact of pill and she should increase her water intake to counter the same.
No need to worry for pregnancy.

There will be no pregnancy in safe period as there is no ovum to fertilise by your sperms then.
her menses would resume in 7-10 days which would be withdrawal bleed from pill and that itself would become her fresh LMP now and her next menses would be a month later after the date of this withdrawal bleed.

so you mean there would be no periods after this withdrawal bleeding on her expected dates, instead they will shift to next month around the dates of withdrawal bleeding.

Also should we go for pregnancy test after withdrawal bleeding if periods didn't come on expected dates? (her expected dates are 25th of month, having 28 dats cycle)

She was feeling heaviness in her abdomen on 3rd feb and today is no pain , is it normal after ipill?

also i heard about ectopic pregnancy.

If she has taken ill on 1st feb then , she should wait for 7-10 days for withdrawal bleed to ensue. Thats mandatory. she will get cramps and symptoms in next few days.
Ectopic can be confirmed only ultrasonologically after the serum beta hcg test turns out to be positive after 7 days of intercourse and if no intrauterine pregnancy is seen. one cannot say it at present.
We should ideally wait for the menses to resume by 8th -10 th feb and if they don't then get a serum beta hcg test. if positive then an ultrasound to ascertain intrauterine or ectopic pregnancy. so that remains the protocol and has to be followed patiently.

she now got period kind of bleeding though they are heavy and she is also experiencing headache, abdominal pain, nausea, tiredness along with it.

Is there anything to be concern about?

hello,
good to hear that she has started with her menses, indicates that she is not pregnant and its her withdrawal bleed. This may last from 4-5 days and slowly would decrease in flow.
however the symptoms she is having are PMS symptoms and may experience nausea, headache, mood swings,low back ache , mild cramps , increased frequency of urine , which shall subside once the bleeding ends.
She can take an antispasmodic like TAB MEFTAL SPAS 500mg twice a day for 3 days to control her symptoms.

1.all calculations have to made from the day of withdrawal bleed now as this becomes the fresh LMP date . The menses due date would be 1 month from this withdrawal bleed date.

2.Pregnancy test need not be taken after the bleed.
3. If periods get late then it should be calculated from withdrawal bleed date .
4. There is a likelihood that next menses can be delayed by 10 -15 days as this is a known side effect or emergency contraception, so be aware and follow barrier contraception after ovulation till menses .

If you are so anxious , then the best way to rule out any pregnancy would be to do a serum beta hcg test which is a lab based blood test, which is very sensitive to the levels of hcg. If the levels of hcg are more than 1, then pregnancy is confirmed , else ruled out completely.

THSI CAN BE ATTRIBUTED TO progesterone withdrawal bleed and symptoms of PMS which may last for few days even after the bleed has ended and may indicate that she may have not completed the withdrawal and may experience some spotting for few days.
This is considered normal. hOwever as advised earlier if is better to get a serum beta hcg test done to confirmatively rule out pregnancy anyways.
