Posted on Mon, 17 Nov 2014
Question: I am XXXXXXX from XXXXXXX XXXXXXX
I Had sexual exposure on 15 th august 2014 with condom but I did oral sex without protection by licking her vagina , I don't know her hiv status, except that oral sex every exposure is with condom only
My last sexual encounter was September 6th
I had HIV 1RNA PCR Test qualitative on 21 st. September which came viral load not detected . Detection limit of test is 70 IU/ml .this test is done 35 days post oral sex and which is 15 days from last sex encounter which is with condom .
Again I tested on 11th October 2014 for HIV 1 DNA PCR Test qualitative and P24 antigen and antibody Which is 55 days from oral sex exposure and 35 days from last sexual exposure which is with condom
These test also came as negetive
Hospital people told that they use 4th genrarion kit for p24 antigen and antibody

Mean while I had pain just below my right side ear which is not continous which used to appear if I get in to cold breeze doctor also said it's because of cold even I have neck pain on right side
No pain on any other side

I am scared about this please help me
If I check online it say test after 90 days itself is conclusive is it correct
And I am afraid of HIV2 but during p24 antigen and antibody test that report says it covers antibody of hiv1,hiv2,hiv1 group 0

Please help I am really scared

Brief Answer:
No further testing required

Detailed Answer:
Hi
Welcome to Healthcaremagic
Greetings of the day

Dear Mr XXXXXXX

Only significant exposure which you had was on 15 th August 2014. The exposure in September was with protection.

RT PCR, P 24 antigen test and DNA PCR test are all sensitive and specific antigen detection test for HIV. They do not rely on antibody production. Negative test done 2 weeks after exposure certainly rules out HIV.
Rest be assured you are not infected with HIV. No further testing is required.

Wats the risk of hiv 2
Is HIV 2 ag /ab test done by me is enough for this as conclusive
Hospital micro biologist said p24 ag /ab is done using 4th gen kit

Brief Answer:
Nothing to worry

Detailed Answer:
Dear XXXXXXX

Almost all HIV infection in XXXXXXX are HIV 1 . Few HIV 2 cases seen are usually rare.

Negative Antigen test for HIV 1 and negative ELISA HIV 2 test done after serological windows period of 8 weeks rules out HIV infection

The cause of your symptoms is not due to HIV infection.
Kindly put back the episode

Take care

Wishing you a very happy and healthy life.

Sir my p24 antigen test technical note read as
This test detects hiv1 antigen (hiv 1 p24 antigen ) and antibodies to hiv (anti hiv1,anti hiv2 and anti hiv1 group I)
In human blood and is indicated for the screening to asses the unknown status related to an infection with hiv1 and/or hiv2. Hiv 1 p24 antigen can be detected usually after 3 weeks of exposure

Test value : 0.05
Cut off value : 0.25
Test result negetive
Method ELFA

I asked microbiologist of hospital said they used 4th gen kit for p24 antigen

Also said 0.05 is not viral load
Please confirm

Also help me if in XXXXXXX is there any facility in any lab for hiv2 test

Can I consider these test conclusive and need not to have any test in future
Is statement of hospital people telling p24 antigen kit is 4 th gen is true because I see in web that elisa has 4 th gen

Should I consider this test conclusive
All sites show conclusive test is after 3 months or 6 months

Help me please to come out of this dilemma
Thanks for your patience reply

Brief Answer:
No further testing required

Detailed Answer:
Dear XXXXXXX

When a individual is infected with HIV , antigen can be detected as early as 8 days following infection. Test like P 24 antigen assay, RT PCR which detect antigen ,negative test 2 weeks after exposure rules out HIV infection. P 24 antigen assay is only for HIV 1 .

Once an individual is infected with HIV, the body starts developing antibodies against the antigen of HIV virus . The time taken for development is 8 to 12 weeks. Test which detect antibodies become positive only after this period also known as Serological window period.

As the test have been negative even after 8 weeks of exposure,it conclusively rules out HIV infection. Antigen testing for HIV 2 is not widely available in India(Incidence of HIV 2 is low ).

HIV one is conclusively ruled so as is HIV 2 with negative antibody test even after 8 weeks of exposure. (The serological window period is usually 8 weeks in majority of individuals and rarely can extend to 12 weeks or 90 days )

Take care
