Is It Possible To Get Pregnant By Having Unprotected Sex On Same Day Of Missing A Pill?

Question: Hi doctor,

I've a doubt and I'm really worried.

My girlfriend missed a pill at the beginning of the new blister (the second pill), after the 7days break. The problem is that we had an intercourse the same day. I explain better: on Tuesday night she took the first pill of the new blister before going to bed (around 1.30), like everyday. On Friday night we had sex and later she fell asleep before taking the second! She took it only the next day (Saturday) around 13.00. As I said she usually take the pill before going to bed (between 23.00 and 1.30, depends on when she goes to bed), so she took the second pill about 14hours late!

On the attached leaf it's written that if you took the pill 12 hours later, the contraceptive efficacy can be reduced and that if you've had an intercourse in the previous 5 days before the forgetfulness, there can be the chance of gettig pregnant.

But it's not over! On Saturday she took her third pill around 0.30 (so the second of the day), but about 3 hours later she had vomit and did not take another pill. Was it absorbed or not?

From Sunday everything has been normal.

My question are:

1) How bad and "dangerous" having take the pill 14hours late? Is like having missed it for 24hours?
2) How long does a pill take to be absorbed? Saturday night pill was absorbed?
3) Is the risk of pregnancy high? (She've been taking the pill for 5 years)

I'm very worried right now because this happened 1 week ago and now it's too late for "plan B"..Is it like she missed two pills in a row or am I just overreacting?

I read your query and understand your concern.
First of all I like to assure you that if your girlfriend have periods beyond 25 days or more the risk of pregnancy negligible. But in case her periods occur 25 days or earlier than the slight risk is there. The ovulation in female occurs 14 days before the next cycle so for a 28 days cycle her fertile period will be from 12-16 days.

In regard to your questions missing pill for 14 hour increases the risk of contraceptive failure but still person remains protected to large extent. Having regular pill is good but missing a pill for 14 hour does not mean it is equal to no pill.

I think pill on Saturday absorbed as it usually takes approximately two hours. She had vomiting after three hours of pill so her pill was absorbed by the time of vomiting.

In regard to your third question I already made it clear that the risk is almost negligible

Hi doctor, thanks for the answer.

I'd like to ask you just few things:

First of all, my girlfriend has her period every 28days and she's very regular (for what I remeber). So, really there is almost "no risk"?

I read somewhere that the pill stops the ovulation and that when you begin to take it, the "rule of the 12-16 days", doesn't count anymore (sorry for my vocabulary). So, if you forget one, the ovulation can start anytime. Is this false? Because in this case I don't understand why is "dangerous" a forgetfulness during the first week (we are close to the break, so to extend this period missing one pill, couldn't determinate an ovulation?).

About the vomit, I'm not sure it was 3hours, she told me she took the pill around 0.30 and that she felt sick around 3 am, but it should more than 2 hours.
So, if 2hours are necessary, why on the attached leaf it's written 4 hours??

Sorry if I'm "annoying", but we're young (23-21) and I'm a bit worried.

I understand your concerns and fear attached with ovulation.

As I said earlier there is no risk and this is true in your girlfriend's case.
The rule of 12-16 does not apply if you stop altogether but missing one dose does not affect the already existing cycle. Even if you stop pill the ovulation occurs after two or more weeks.
Other than those Pills do inhibit the fertilization and implantation of ovum in uterus (important steps in pregnancy), so occasional ovulation does not affect much about the risk of pregnancy. I mean to say that still pill prevent pregnancy if continued and fortunately your GF continued it after missing single dose.

I understand your statement about 4 hours of absorption time and it is possible because throughout world different combination of hormones available as a contraception pill. Despite those three hours is enough time to get about 70-80 % of absorption. In my view 70-80% of standard dose is enough to prevent pregnancy and again I assure you that there is nothing to worry.

The best answer to your question is because there difference in between the practical and theory. Being a doctor I know many side effects for any drug but that does not mean every side effect occurs to everybody.

It is true that there is risk of pregnancy if contraceptive pill not used regularly(theory) but is is also true that risk is negligible (Practical).
