default
Posted on Wed, 22 Feb 2017
Question: Recently I visited a plastic surgeon who is a family friend . I discussed my condition with him and he has suggested to inject some medicine into my scar. Please suggest should I go ahead with it or wait for some more time. I had surgery in May 2016 but the scar formed in August 2016. Sometimes the pain in scar is unbearable. Hardness/swelling is scar is very slowly improving.
Please suggest whether I should go ahead with the injection or should I wait.

Brief Answer:
Better to wait.

Detailed Answer:
Hi.
Thanks for your review query.
Noted the history well.
Since the surgery was done in May 2016 and scar formed in Aug 2016 it is about 5 months that the scar is still not mature.
It takes about 9 months for maturity hence I would
advise you not to inject anything at the moment.
Please continue the medications and should help.
I hope this answers.

Hello Doctor
I am worried because sometimes the pain in scar is very unbearable.
Two of my known friends also had fitulotomy done and according to them they were alright in 2-3 months. Apart from you the doctors i have consulted say that it should not take such a long time to heal. That is why i continusouly seek ur advise which is in accordance to everything i have read about scars i.e it usually takes 1 year for a scar to fully mature.
Request you to please suggest that whether the healing time is different from a person to person. Also if you could please confirm if any other patient you have treated also had problems with the scar and did it eventually heal.
Thanks for your support again.
Regards

Brief Answer:
As detailed below.

Detailed Answer:
Thanks for your appreciation.
Yes, the scar tendency of each person is different and no one can really predict who will respond in which way.
I can understand the plight of pains you are going through.
It is rare to have such pains but there is a sizable number of patients who have had similar problems that eventually settled, so will definitely occur in your case.
Please continue the medicines and you may need alter the dose of Neurogaba - how much are you being prescribed and how many times?
Do Sitz bath help?
What are the factors that help to reduce or increase the pain?
What is the type of pain?

The scar is a little bit swollen/raised at the place where the incision was made and the adjoining skin is flat but i can feel it is hard. Is it normal
Sometimes the raised portion of the scar becomes very tight and pains a lot. Sometimes after the bowel movement i can feel the pinching pain on the adjoining skin which is flat but hard.
Taking rest relieves the pain and mostly exhertion increases the pain.
Sitz bath helps in relieving the pain and also helps in cleaning the anus.
Would also like to mention that 2 months back i had some fungal infection in the perianal area which was treated by onabet b cream. So i discontinued the cream.
Please note that when my scar and the adjoining skin pains / irritates ,applying Onabet B cream is really helpful. Can it be possible that still the fungal infection on scar is present.
I am taking 1 capsule of neurogaba at bedtime.
Please suggest .
Thanks again for your support.

Brief Answer:
As explained below.

Detailed Answer:
It is normal for some fibrosis to be hard as per the natural tendency of an individual.
Possibly the nerve entrapment or micro-bleed can cause the hardness and pain/tenderness.
Usually, the fungal infection does not grow on scar tissue - it needs wetness.
If taking Neurogaba once is controlling your pain, it is fine- if not consult your Doctor whether to increase to twice a day on a trial basis.
Onabet b contains steroids too and thus helps in relieving the symptoms.
