Question: Hi,

I am 30yrs OLD, I am not able to control my thoughts at all, I have problems in focussing on one thing. I always stay in depression and anxiety, i shiver when i have presentations and I avoid groups. I am always thinking not doing.. The best i do is to relax i Masterbate. Even i don't have a control on masterbating. For the last 2 yrs i reading magazines, watching youtube videos all those stuff i doing but i can't control myself, I get angry very quickly i stay silent at that moment, when i come home, all thoughts trouble and i don't sleep. I am an insomnia patient i am taking homeopathic medicines for the last 2yrs. I am not always in a relaxed state of mode. To make my self busy I play Violin and Photography. If i a have long holiday i plan for treks as well. The problem is the daily life, how to deal with it. For me everything as a problem, parents, relatives, friends,teachers, even myself. Please suggest me some daily mental exercises which include Violin any my studies and i play Table tennis. Actually,I am PhD student working on malaria vaccine. I want to be free and focussed in all disciplines. So, kindly help me out, I am taking Vitamin B12 and B6 supplements, zincovit and healthy diet i am following. Please help me out with my situation.

Thanks for writing to us.

You have very well narrated your problem.You seem to be in excess stress since long.
You have adapted wrong cope up mechanism of masturbation to relieve the stress. This needs to be stopped with help of counseling.
Masturbation itself is not a problem but to use it as stress relieving mechanism is not healthy.

From your history three differential possible diagnosis come in my mind.

- Depression with anxious distress

- Generalized anxiety disorder

- Attention Deficit Disorder (ADD)

For confirmation of diagnosis detail history and mental status examination are needed. That is done by psychiatrist in routine.

You need to learn stress management as well as relaxation techniques.

Specific treatment of depression/anxiety includes both medicines and cognitive behavior therapy(CBT).

In mild cases only CBT is sufficient. In moderate to severe cases initially medicines and then CBT is added. And later on medicines stopped gradually.
If medicines alone are used chances of relapse are more after stopping the medicines.

In medicines SSRI group of drugs(fluoxetine,paroxetine,sertraline, escitalopram) are main stay of the treatment. I prefer paroxetine. Benzodiazepines group of drugs(clonazeapm, lorazepam or etizolam) are used initially for short duration for fast symptomatic relief.

If diagnosis of ADD is made other activating medicines are needed.

These conditions are treated by psychiatrist. Medicines I mentioned are prescription medicines. You need to consult doctor for the same. Ir request you to consult psychiatrist personally to get rid of your problem.

I am pretty much convinced but, can u please elaborate more on what kind of test they conduct for "Mental Status Examination" and ADD( what happens in this case).
Any ways I will try to consult a psychiatrist.

Mental Status examination (MSE) includes detail interview as well observation by a psychiatrist. It includes many questions to assess mood, speech, affect, psychomotor activity, judgment, memory and so on.

Attention Deficit Disorder (ADD) is a psychiatric condition in which person has difficulty in concentrating at a particular task, restlessness, and irritability and secondary occupational or social disturbances. Many times it is there with depression also. History of similar symptoms in childhood should also be evaluated to reach diagnosis.

Dear sir,

I am having the exact symptoms of ADD. From childhood onwards, keeps on getting accumulated, My parents even thought it will go by age but it is permitting and accumulated and also 1 yrs back my arranged marriage engagement was cancelled, i was not liking that girl, they backed out. I am coming out, and explaining myself to everybody mostly my parents only. Thats why aim taking homeopathic medicines since year now. Social disturbances is too much,, relatives as well as friends. Stress is too much, if i say my parents then just say leave violin, photography, just conc. on studies.. but only studying does not help me. I am actually servicing b'coz some kind of other activities, iam taking pleasure from it. My parents listen to me but I am in XXXXXXX and all my relatives are in XXXXXXX They keep on telling my parents they get both down, always a problematic situation at my place. My parents are financial well settled here in XXXXXXX my dad is in ministery. I don't have any bad habit, smoking or alcohol. But thoughts of relatives just triggers me, i am just helpless how to handle them i am not understanding.

Thanks for information.

There is thin line between depression and ADD. When mood of the person sad or low most of the time of the day possibility of depression is likely and lack of concentration is part of depression symptoms.
It is good that you do not have any habit of alcohol or smoking.

I am not sure that homeopathic medicines would work or not in this case. But of course no need to feel helpless as consulting a psychiatrist will help that is for sure.

Your parents also need to be counseled I guess.
