Question: Run a theory past you if I may about my bladder issues. It's causing me panic attacks every morning and ruining my days

I've been training my bladder to empty when not full for months to avoid being in the situation I need a wee and can't go, mainly work related. So now my bladder thinks it needs to empty or at least my brain does. Two weeks ago I did a run and didn't drink before it, I also urinated twice when I didn't need to. When I got home with a lot of codeine in my system (I take for IBS) I felt the muscle that controls your weeing value spasm a bit, nothing painful and totally a result of codeine and or lack or drink / loads of sweating . Anyway this made me focus on my bladder and were as normally I would of just wee'd all the time to keep myself empty I started to noticed it and think it wasn't normal. This causes panic, panic causes more urine to be produced. If I had something wrong a it would have come up on the tests (been the doctors" had bloods) and b I would be waking up in the night. The hesitation I get isn't actually hesitation, everyone needs a second to release the muscle to wee, hesitation is having to push, stop starting or not finishing which I never have. So to summarise, I need to train my bladder to go every two / three hours and if that means parking up or leaving a meeting then so be it my health comes first. I'm going to print out some reminders of this to out around the house to help with the morning panic

I can understand your concern. Some individuals develop issues related to urinary bladder and this cause changed frequency of passing urine, dribbling, reduced flow etc. But either some urinary tract infection (UTI) or some anatomical abnormalities cause such symptoms. You are just 33 years old and I dont think prostrate is the cause of your symptoms. You have mentioned that you have to train your bladder to pass urine every two three hours to avoid situation where you need to pass but you can't go.

One thing I would like to know in follow up, have you underwent the tests for urine? Blood tests, anatomical tests at least USG, Urodynamic studies etc are needed in my opinion. If yes then what were results.

You have history of panic also. The intrusive desire to pass urine at fixed time might occur due to anxiety related to the situation where you can't escape. This is totally imaginary situation but cause the symptoms like you have to wee every two three hours. I would like to know does your mind know that all these things are wrong and still cant control it?

Please provide me details I have asked in follow up and also correct me if I am wrong at any point.

I do have a history of panic, I've had tests to the point my doctor is confident I have no medical issues. My mind won't relax so I put my theory together.

You have no medical issues and have history of panic. This supports the diagnosis of anxiety behind cause of your symptoms. Most likely the intrusive thoughts that you have to pass urine otherwise how would you manage in cause of escape is difficult are related to obsessive thoughts. Going to pass urine is compulsive behaviour behind this. These thoughts are dominating in your mind and are causing significant distress and even you are aware of the fact that there is nothing wrong medically in you.

In my opinion training the bladder is not a solution for this problem. Training is needed in medical issues but most probably you have psychiatric issues. In such symptoms medicines as SSRIs can help you. Fluoxetine, Paroxetine etc are commonly used medicines. These medicines will reduce the unwanted thoughts and will relax you. The anxiety and panic will also decrease. The medicines if used regularly help to reduce the symptoms in most of individuals.

Apart from this Behaviour therapy with relaxation exercise can help you. Proper psycho-education and reassurance that there is nothing wrong in your case will help to improve the condition. Thought stopping technique and progressive delaying the emptying of bladder will reduce the unwanted thoughts on long term.

So please don't remain stressed. The problem in your case is treatable, you have to visit a psychiatrist for treatment.
