Is Coronary Angiography Advisable For Patient With Positive TMT Result?

Question: Annual health check up has shown TMT mild positive in 2014 and 2015, in 2013 it was negative. Also triglyceride level was 278 / 311 in 2014 & 2015 respectively. HDL is 33 and LDL 100. BP 140/84. I was prescribed medicine for Triglyceride in 2013, 2014 & 2015. Blood test from Thyrocare shows Triglyceride little lower at 258 in Feb 2015. Further I was advised Angiography. I went for second opinion to another hospital but the cardiologist there did not advise medicine or angiography. Family history - father has hypertension since 30 years & is 86 now. I don t have diabeties/other ailments, I do get asthamatic allergy in Feb-March every year. I have not started any medicine because of differing opinion of doctors. I walk daily 30 minutes in morning since 30 years. Kindly give your advise.

Based on your positive TMT results (inferio-lateral ST depression), I would advise you to perform a coronary angiography.

If you refuse to perform this examination (for any reasons), you should perform a coronary angio CT scan.

Because having a doubtful medical history and an abnormal blood lipid profile, coupled with these TMT results, you are considered at intermediate probability test for detecting coronary lesions by this imaging coronary study.

Another alternative would be a nuclear provocative cardiac stress test (CARDIOLITE).

Through coronary angio CT your attending physician could investigate your total coronary Calcium Score (investigating that way the probability of future coronary events) or visualize your coronary arteries (thus creating a satisfactory view of your coronary artery patency.

Meanwhile it is recommendable to continue the proper medication for your lipid blood profile coupled with a healthy diet and daily physical activity.
