Is Chest Pain A Symptom Of High Blood Pressure?
Question: Hi, I am a 66 year male and am diabetic. over the last 6 months or so I am also suffering from high blood pressure. My diabetes is fairly under control ( have been taking metformine for around 12 years now and my blood sugar is around 120 to 130 in the morning). I have been taking CTD 12.5 and my pressure has been pretty OK....between 135/80 to 145/90 sometimes. Recently, last week, I started getting a pain in the middle of my chest. Worse when lying down. My blood pressure had gone up to 180/110! I hadn't been doing anything different or unusual. My doctor did an ecg and said it didn't look bad. He prescribed something for acidity but also Prolomet XL 25, one a day in addition to the CTD 12.5. Am also taking PNZ 40, one a day. As I am writing this I have a dull but strong pain in my chest. My bp reading is 180/100. I have just taken my prolomet and hoping it will help. Grateful for your advice. Thanks

I have read your query and understand the concern.
1) If your blood sugar (fasting) remains between 120 to 130 mg% then I would say it is under well control.
2) As you had dull chest pain since last 1 week and also your blood pressure has gone up to 180/110 with your regular anti-hypertensive medications (which is Chlorthalidone).
We can suspect here that your chest pain could be the reason for your blood pressure rise recently which in turn could be either musculo-skeletal or related to acidity (as your doctor suspected) or may be cardiac origin but the chances of cardiac origin is unlikely here since your doctor said your EKG is not bad (say normal here).
In my advice,if you were my patient I would like to give,
1) Over the counter pain killer like diclofenac ER 100 mg or ibuprofen 600 mg with continue taking pantoprazole (PNZ-40) twice daily for symptomatic relief.
2) Digene gel or syrup may also be helpful if you have heartburn.
3) Take prolomet-XL 25 mg (as your doctor prescribed) which probably will lower down your blood pressure back to normal.
4) EKG can also be repeated on the next day for a safer side.
If still,BP does not come under control then Amlodipine 5 mg after consulting your regular treating doctor.
I would expect that your dull chest pain will start improving with this symptomatic medications within 2 to 3 days with your blood pressure with another anti-hypertensives (apart from CTD-12.5)
Or in case,if not then you should be further evaluated for repeat EKG,chest X-ray,2D-echo of heart and TMT if blood pressure is under control.
But as I said earlier, the chances of this dull chest pain due to cardiac origin is very unlikely.So,no need to be worried much.
