Question: HI I am 23 years old (male) height 5feet 8inch , weight 68kg. I am so lean since my childhood but when i turned 21 i observed that i am getting fat on my chest , bums, stomach. My nipples bulge out sometimes. I tried gym for couple of months but they came back much bigger this time. I recently met a doctor and was prescribed to run the blood tests and an mri scan on my chest. The doctor said that my testosterone levels are fine but had gynecomastia(small breast tissue) and suggested to meet the plastic surgeon. My bums are turning out to be loose and fat like women bums. Should i have to go for surgery? or are there any alternatives?

If you are having a normal erection and your hormonal profile is normal, then it is only a matter of cosmetic appearance. Unfortunately, there are no medicines to make the gynecomastia disappear. Keyhole surgical procedures are available with quite good cosmetic results.

Ultimately, it's your call as to whether you are ok with the present appearance or not. So please decide accordingly.

is my bum fat an effect of Gynecomastia? I have loose skin all over my upper body and bums. if i choose to go for surgery will there be any side effects and guarantee that it would never come back

If you can upload your hormone profile, I would be able to comment on whether the loose skin is due to a hormonal issue. Gynecomastia can occur in some young men without any obvious hormonal abnormality.
The breast tissue can be removed in toto in which case there will be no recurrence.

I had normal erection but my penis is small and has Premature ejaculation problem. My physicality is small(I look like a 16yo though I am 22). Will these also become a concern in my case along with gynecomastia and bum fat.

if not what surgery would be ideal to go for in my case, can you suggest any? you previously mentioned keyhole and toto surgery's.

I am not sure whether i had premature ejaculation or erectile dysfunction, because i never engaged in sex , my erection will last about 3-4 mins maximum if i masturbate. Just to be clear.

Welcome back.
The key hole surgery has a smaller scar when compared to the open surgery. The whole of the breast tissue is to be removed so that there's no risk of recurrence. The surgery is only for cosmetic purpose and is not a necessity.
If you have normal erection, you are potent. The erection lasting 3 - 4 minutes is sufficient if it is terminated following ejaculation.
It is important that you are relaxed as stress can interfere with erection and ejaculation.
