Is Blood Sugar Level Of 296 After Eating A Cause For Concern?

Question: I never have had diabetes, my blood sugar level on a complete metabolic stated 100 ml. sometimes lower. The last report in 2013 was 105 ml, I always was in good shape normal weight but have gained 25 lbs since 2004 (2004 turned 50) until reaching 40, I ranged in weight during my 20s 30s 115, by 40, 130, now in 2015 171 lbs at 5 foot 5. I hike fairly regularly 6 miles three times a week, and eat well. I do not smoke, but since my parents' death and our large estate and I am a PhD with academic responsibilities, I drink quite heavy red wine. My age is 61 years old.
Yesterday, I received a urinalysis as I need a preop for a simple surgery on removing a granular cell tumor, presumably benign, on my labia majora. Just had an ekg comes out normal. But one irregularity in the urine analysis is I had 3 + >1000ml/dl. I immediately went to my primary care doctor, and along with taking the ekg for the pre op, I asked her to run a complete metabolic panel. She asked me to go to their lab as she wants the results based on the urine analysis. I responded usually I fast, she said not necessary. In between without her knowing I asked the tech to give me a prick test, and the glucose level after eating organic oatmeal with almond milk and sprinkles of almonds was 296. Never in my life I had high glucose levels although on my father's side (Lebanese quite common) we have diabetes, including paternal grandmother and aunt. Dad never did as he was slim, ate sensibly and healthy, but developed at 87 years old after chemotherapy for lung cancer. Sorry for the long history, but am concerned.

2. You said to have measured your blood glucose level after eating organic oatmeal with almond milk and sprinkles of almonds and it resulted 296.

To me, this is not alarming or stressful. Let me explain why:

Blood sugar levels (bsl) do oscillate due to many factors including foods, hormones, stress, etc.

We do usually recommend to measure fasting bsl as we want make sure one patient does not have diabetes.

When you eat, it is normal for bsl to get raised (even higher than your reading of 296). This is for the first 2 hours after eating, time which is needed to secrete insulin and to make this sugar get inside the body cells. Consequently, the bsl will be normal again after 2 hours of eating.

At this point, I'd like to know when did you do the testing : was it 1 hour after eating? 2 hours? or less?

This is important to explain your figures.

If it was less than 2 hours, then, wait until it will be more than 2 hours after eating oatmeal and repeat the testing. Meanwhile, you are allowed to drink only water. This is called post-prandial bsl.

I also advise to repeat the testing after fasting for at least 8 hours (can drink only water) and ask your doctor to run HbA1C test (which measures the average of bsl for the last 3 months). In this way, we'll make sure you do not have diabetes.

3. Please take note that we do not make a diagnosis based only on one measuring. We evaluate many factors until we confirm that you really have diabetes. So, no need to worry unnecessarily.

Hi my dear,

Thank you for providing UA report.

As I was going through it, I noted the abnormal presence of glucose (sugar) in urine.
I still think that is not alarming as long as you did the testing non fasting.
I advise to repeat the testing, but this time fasting and compare the reports.

It is also normal to get glucose (sugar) in urine if you have eaten prior to testing.

If I was your caring doctor would advise to:
- repeat urine analysis fasting
- run blood tests fasting including HbA1C level

After evaluating such results, we can determine/exclude diabetes. Cannot judge on an UA report only.
