Is Berirab Injection Necessary For Grade 2 Dog Bite?

Question: My Son (6 year old)was bit by street dog. There is one deep wound(puncture wound) at thigh and 3 to 4 scratches at same place. we took him to the hospital. doctor has given "RABIVAX" injection immediately and then cleaned the wound and other affected areas with the tincture and given the other dosages after 3,7,14 and 28th days. wound was actively bleed ed for about 6 hours (not heavily) after that stopped.
we identify the dog and it haven't bite any one in last few years and i think it is not a infected dog.
doctor said, if the dog is infected or it is bite more people, than we can go for "Berirab" injection.
please suggest us, is that treatment is fine or we can go for the "Berirab" injection. my son's weight is 18KG's so doctor suggested to take two dosages of Berirab injection, if we required to go for it.

Your son had grade 2-3 dog bite wound on leg. The immediate treatment is cleaning the wound under running water. Rabies immunization then follows within 48 hours.

As I understand your doctor has into consideration the depth of wound and infectivity of dog to decide regarding inj berirab. But berirab best if given within 7 days of bite. In your case since it already more than 28 days and dog looks non infected, I don't think berirab would be useful and appropriate now.

Sorry, I have not given date of dog bite. it was on May 22nd 2014 at 7 pm in the night that is yesterday. only one dosage of RABIVAX has give.

please let me know, whether the doctor has take proper care. is it advisable to go for Berirab now.

Treatment given by your doctor is correct. But if wound is grade three (i.e. if skin is torn and blood has come and it is moderately deep ) then it is advisable to take inj berirab in all such wounds regardless of which dog bite him. Half dose in wound and half intramuscularly (20 units/kg). Discuss with the doctor and let him have berirab also.

when Berirab have to be given after the bite. means, is there any time limit with in 24 hrs or before 7 days.

Berirab should be given within 7 days of dog bite. Beyond a week, it is not useful as antibodies start developing because of vaccine
