Brief Answer:
Seems like an infected cyst

Detailed Answer:
Hello. Thank you for writing to us

I am Dr.Kakkar (Dermatologist). I have gone through your query and I have also reviewed the Image.

I can see a swelling with a redness. Could be an infected Sebaceous cyst Or Pilonidal Cyst

I would like to know that is it painful? warm to touch? does he have fever?

I would suggest an Oral antibiotic e.g Amoxicillin Or Cephaelxin, thrice daily, for a week along with an Oral antiinflammatory e.g Ibuprofen, twice daily, to take care of pain, swelling and inflammation.

Regards

Brief Answer:
Oral antibiotic and oral pain killer

Detailed Answer:
Hi.

The area is definitely infected and I therefore suggest a course of an Oral antibiotic to take care of infection, pain and swelling, at least for a week or 10 days.
In addition he may take an oral antiinflammatory e.g ibuprofen, twice daily, for a few days for symptomatic relief from pain.

Regards
