Is Voltaren Effective In Managing Pain When With Barrett's Esophagus?

Posted on Thu, 4 May 2017
Question: I have Barretts Esophagus and take Effexor XR 150mg daily. My pain doctor is telling me to use Voltaren Topical and said to take "an anti-inflammatory ", but not every day. This sounds iffy given my stomach condition I have also read that Effexor + anti inflammatory (Nsaids) is a no go because of bleeding risk.

Brief Answer:
Anti inflammatory can be taken on as and when required basis.

Detailed Answer:
Hello!

Welcome and thank you for asking on HealthcareMagic!
I carefully passed through your question and would explain to you your query.

Venlafaxine is an antidepressant in a group of drugs called selective serotonin and norepinephrine reuptake inhibitors (SSNRIs).One of the side effect of this drug is that it has mild anti platelet activity.
As per your question yes anti inflammatory medicines inhibits cyclo oxygenase enzyme and decrease the platelets count.
However as and when required doses of anti inflammatory in my opinion will not put you on very high risk.
However if you will use anti inflammatory regularly it may potentiate the anti platelet action of Effexor and bleeding may occur

Advice-
In case if you have to take anti platelets frequently monitor platelets every week.

Thank you and wish you a great health.

Therein lies the problem. Stopping hydrocodone would require regular usage. Perhaps a bit more information is in order. I have a chronic pain condition secondary to two cervical fusions. Both surgeries were delayed, one because XXXXXXX XXXXXXX AMC gave the disc rupture a low priority (quoting my neurosurgeon at the time "no one ever died from a ruptured disc"). I waited 7 months after the diagnosis before the surgery was performed. The second surgery was delayed (ridiculously so) by Tricare. They would not authorize an MRI for 8 months, demanding instead that I to physical therapy for 6 weeks - that made things worse. They then denied again demanding more PT. Finally, 8 month later the MRI was authorized. It revealed, quoting my neurosurgeon, "one of the worst ruptures I have seen, your neck is very unstable"
My last back MRi showed two bulging discs and L5S1 desiccated and bulging. Bottom line - confirmed by my second neurosurgeon - nerve damage in both locations. Ortho surgeon consult revealed the same. Since then the nerve pain, which in my experience is unaffected by Nsaids - has increased in both areas. Thanks for your opinion, but I am unwilling to take the chance. Anti inflammatory drugs have also been linked to heart attack and stroke. Honestly, i am not satisfied with your brief answer. Adding anti platelets with a drug with proven risks is unacceptable - weekly blood tests, really? Don't think so. DONT NEED ANOTHER REPLY, JUST REFUND MY $15.

Detailed Answer:
Hi again,
I am sorry for your problem.

As per your query yes anti inflammatory drugs can increase the risk of Heart attack and stroke. But if we follow books in every case then believe me we can't use any medicine on patients because potentially harmful side effects are noticed even with vitamins also.
We can't follow books blindly in every case Practical medicine is something very different from theoretical medicine
As per your nerve pain is concerned i would suggest you to discuss the option of pregabalin with your primary. They are very effective in treating neuropathic pain.
While treating a patient a doctor has to consider pros over cons.
Almost every medicine has a direct or indirect interaction with other medicine. It's the work of doctor(Not Google) to decide whether the combination is safe or not.

My advice is you can take anti inflammatory on as and when required basis.

Regards.
