Is Telmisartan The Right Medicine For High BP?

Question: HYPERTENSION
Doctor, My father is 73 year old. He has diabetes 4 years back only. Its well under control all these years.He has hypertension since a long time back and taking Atenlol 50 mg with stamlo 5 mg since a long time. His BP is quite normal oscillates between 110/65 to 130/75.On
He told me that he is quite comfortable with the present medicine. One day on his own he went to a doctor and enquired from him whether he has to change Atenlol and stamlo since he was taking for a long time. The doc prescribed Telmisartan 40 mg and asked him to stop Aten and Stemlo forth with and told him that this new medicine will take care of the kidneys also as my father is a diabetic. He was taking this new medicine since a month. He was monitoring his BP with digital aparatus and found that his bp was always oscillating between 140/90 and 160/90.Never it has come down to120/80.Now he is thinking that he wants to go back to ATen and Stamlo as previously as he is comfortable with that for many years. Kindly advise whether he can switch over back to the old medicine or the dosage of the present medicine to be increased. Will there be any harm if he does like this as he was taking Telmisartan only for one month.My father was a bit reluctant to go to that doc to talk about this as he may be annoyed.

Telmisartan is a good choice of medication to control hypertentesion, those who have diabetes along with it and its a protective drug for kidney.

For my patients with similar complaints I prefer to Check BP manually and confirm values, And if its high, stamlo 5 mg along with Telmisartan 40 mg is preferred.
After addition of stamlo 5 mg needs to monitor BP.

Since your father 's BP is not well controlled, Better to continue Telmisartan and we can add stamlo with it , better to consult doctor for manual measurement of BP and additional prescription.

Doctor,thank you very much for your prompt reply. From your reply i inferred that you are opting out Atenlol instead, you are preferring Amlodipine along with telmisartan. I too felt like that after studying my father. I too am doing 4th year of MBBS . I feel he can take Telma (40mg) Am (5mg) instead of pure Telma.What do you say? He is very lean since his younger days , so i feel he doesnt require a diuretic.Do you agree with me?

Combination medication can be given

Hi, thanks for follow up query.

Combination therapy is good considering compliance to medication.
2nd line of management after ARBs is diuretics, Body weight is not going to affect diuretic therapy.

Telmisartan and amlodipine can be given together.
