Is Provera Better Than Prometrium?

default
Posted on Sat, 15 Nov 2014
Question: 58 years old, have had 4 biopsies (all normal proliferative, secretory now atrophic) last one end of XXXXXXX 2014 (atrophic), hysteroscopy, D&C, endometrial ablation, polypectomy (all in March 2010), 2 sonohysterograms, upteen pelvic ultrasounds, blood tests. Gyn said blood tests (end of June) show I am NOT in menopause. Endo thickness 7mm end of XXXXXXX 2014 after 5 1/2 months steadily on Provera 10 mg/day. She is now telling me to go on Provera 10mg x 14 days every 3 months to keep lining thin. I am TERRIFIED of having a HEAVY bleed again and the side effects of the Provera so, asked for Prometrium (family doc prescribed it but, I asked gyn before taking it). Gyn says she prefers Provera to thin lining as Prometrium is not as effective. But, I had a horrid bleed that landed me in Urgent Care last year while on the Provera. Who should I believe as I do NOT want a heavy bleed again like I had before on it?

Brief Answer:
WEIGH YOUR OPTIONS, CONSIDER MIRENA

Detailed Answer:
Hello
Thanks for writing to us with your health concern.
Provera IS more effective than Prometrium, however Prometrium has very few side effects compared to Provera.
Regarding the heavy bleed that you had even while on Provera, that can occur once in a while in the perimenopausal age group, and the likelihood of such a horrid resistant bleed increases if you take PRometrium
ULtimately the decision is yours.
If the side effects are highly unpalatable, you can opt for Prometrium, else go with Provera.
One more product can be considered - the Mirena IUD - intra uterine device.
That is also optimal for thinning the lining, while averting most of the side effects.
Do discuss this option too with your GYN.
All the best

Doctor, I cannot have the Mirena as I have had an ablation and there is far too much scar tissue to insert it. My current gynecologist has ruled this out.
Are you saying that PROMETRIUM (the more natural progesterone) will produce a heavier bleed than the PROVERA? Both of my doctors have said it's the opposite way around???? I'm confused. Please clarify. It is the heavy bleeding that I am most concerned with having again while on Provera.

Brief Answer:
PROVERA IS MORE EFFECTIVE

Detailed Answer:
Okay if the insertion is tough, Mirena is ruled out, but it can be tried under anesthesia.
Just a thought.
Well, let me put it in a better way.
For thinning the lining of the uterus, Provera is more effective than Prometrium.
Which is repeating precisely what your GYN said.
Hope it is clearer now.

Sorry, I am at work and trying to flip screens here.

Ok, so the Provera is better at thinning the lining. Is it likely to cause heavier bleeding than the Prometrium or both about the same?

THAT is my concern...the heavy bleeding caused by the Provera.

Brief Answer:
EXPLAINED

Detailed Answer:
That is okay.
Yes, Provera is more effective at thinning the lining of the uterus.
When taken cyclically as recommended by your GYN, it is more effective at inducing regular normal bleeding than Prometrium.
It is less likely to cause heavier bleeding as against the Prometrium.
Take care.
