Posted on Mon, 4 Jul 2016
Question: Have been on Paxil for 10 years & it is not working for me anymore, doctor wants me to try Fetzima 40mg daily, is this a good choice, what about going off of the Paxil, stop completely or taper off for a few days?

Hello, and thanks again so much for using my direct private service.

I understand that you have been on Paxil but it has lost its efficacy. You have been given Fetzima or Prozac as suggestions for a replacement antidepressant.

I would probably favor Prozac for you. It is a very reliable and very effective medication. Fetzima is less well known, but would also be an effective choice. One advantage of Prozac is that it has a long half-life, so by starting it immediately you could taper off the Paxil more quickly, though in either case you need to do so carefully to avoid withdrawal symptoms.

In summary, I think either would be a good replacement for you, but would favor Prozac. Paxil cannot be stopped abruptly and needs to be tapered carefully.
