Is Nepro HP Safe For Treatment Of Kidney Disorder?

Question: Dear Dr.
My 75-year-old widow sister-in-law, ckd patient was on dialysis for five years as both the kidneys were not functioning following left knee replacement surgery. Finding significant improvement the doctor stopped dialysis and advised periodical blood exam. It has been more than one year since then.

However, her right knee was also replaced at the end of fourth year of dialysis which went off smoothly and without any problem. Her hemoglobin level was between 7 to 9, creatinine 3.5 to 4.5, sodium and potassium levels were also more or less within limits.

Recently she had a fall from her sitting-stool and fractured her left hip. She underwent hip replacement also in the last month and since been recuperating.

She had slight mental problem following the death of her husband at the end of last year on whom she was very dependent. Psychiatrist prescribed her Qutan 25, Zosert 25 and Admenta 5 once 1 tab daily.

Discharge advice following surgery was:
Cetil 500 1 tab twice for 10 days
Pantocid DSR 1 tab bbf and bd
Pyregesic 1000 1 tab thrice daily x 10 days
Vigylac 1 cap twice daily
Bio D3 XXXXXXX 1 tab once daily x 6 weeks
Cilacar 20 1 tab twice daily
Tide 5 1 tab once daily
Feburic 20 mg 1 tab daily
Qutan 25 1 tab daily
Zosert 25 1 tab daily
Admenta 5 1 tab daily
Uprise D3 1 tab weekly
Nodosis 500 1 tab x twice daily
Inj. Clexane 40 s/c once x 5 days.

After intake of above medicine she was found extremely drowsy for which she again hospitalized for 3 days. Following several exams she was found to be suffering sleep apnea. Qutan 25 was also withdrawn on the advise of the psychiatrist and nephrologist.

Her final medicines:
PAN D 40 twice daily bef meal
Vigylac SOS
Bio D3 XXXXXXX 1 daily
Cilacar 20 1 BD
Feburic 20 1 daily
Zosert 25 1 daily
Admenta 5 1 daily
Nodosis 1 BD
Orofer XT 1 BD
Inj. Mircera 5o s/c fortnightly

Pt. is stable, bit hypertensive, but refuses food orally. Takes very little daily. Following this she is being given Nepro HP renal food supplement twice daily. Passes urine 900 to 1100 ml urine with semi solid stool against intake of 1200 ml water.

Is it safe to use Nepro HP or how her food intake can be improved.

Yes. Nephro HP is good supplement,

Improvement in diet is necessary. It can be done through:
1. Small multiple meals
2. Low potassium diet. Avoid nuts, avocada, banana, etc.
3. Enough calories. Mild exercises will improve appetite and activity is necessary.
4. Family involvement, social activities, should be helpful.
5. Sleep is important. Should be regular and at least 8-9 hours at night.
6. Salt and water restrictions. At present good. Continue the same.
7. All other medications can be continued. Need to watch for anemia.
8. Avoiding infections, particularly urinary tract is necessary.
9. Watch diet closely for any nephrotoxins
10. Due to associated dementia and weakness, risk of injuries,falls etc present. Caretaker should be around to avoid risk of falls.

Nephro HP is safe. Continue along with above and due to associated dementia, family support and involvement in social activities is important.

Urinary tract infection presents with burning urination, increased frequency of urination and fever. It could be all or one of the symptoms.
When in doubt, get the urine examined for leukocytes and bacteria.

Medication depends on culture and sensitivity of urine sample. Your doctor will prescribe an initial antibiotic and wait for culture report to start appropriate antibiotic if necessary.
