Is HIV Transmission Possible Through Saliva?

Question: Dear Doctor,

I hope you are doing well. A couple of days ago I went to a strip club. The stripper pulled out my penis and began to give me a hand job. In the process she spit on my exposed/uncircumcised penis twice. I am concerned that her saliva may have had blood mixed in or she had bad oral hygiene. Furthermore, she may have had someone else's semen on her hand. I am concerned that the any or a combination of these agents/fluids may have touched cuts on my penis (don't know if I have any cuts) and/or it may have touched or entered my urethra making me vulnerable for STDs, namely HIV.

I should note that people in that area widely use heroine and cocaine. I don't know about the stripper, but sadly she seemed different.

I sincerely appreciate your thoughts. I am really scared here and this is not typical activity for me. Should I get tested?

- There is no possibility of transmission of HIV to you since oral sex has least possibility of HIV transmission. Because saliva contains substances that prevents multiplication and so survival of HIV.

- Saliva do not contain blood in it so there is no possibility of HIV in that case also.

- If it is a semen of other person in hand and cut on the penis than there is possibility of HIV transmission. But overall possibility of HIV is least in my view.

Thank you so much. What if there was a strain of blood in the saliva due to a bite, gum disease, etc? My concern is the possibility of a small cut on my penis or it getting into the urethra.

Hello Again Doctor,

I sincerely appreciate your advice, means the world. Since I am uncircumcised with presence of foreskin if there were blood in the saliva or the hand of the lady, would I still be at risk. I know HIV dies quickly in open air/outside its environment, but I don't know how fast.

Again, I did not ejaculate and I did not allow the lady to finish out of fear. My penis was exposed in open air for maybe two minutes XXXXXXX and then I put my pants back on. I regret taking two hours to wash my penis area.

Does the additional information make things more risky?

Again, I am sorry about the repeated question. Need some mental help!

- You are anxious and stressed about the incidence. The fear of the HIV after incidence has not gone from your mind. You should forget about the incidence and have to make your mind calm and tension free that you are not HIV negative.

- You can test your self if the stress is not relieved for HIV proDNA PCR, it is gold standard and definative test for HIV.

- As I mentioned earlier oral sex has least possibility of HIV, even if it has blood by bite the saliva cause dilution and make transmission ineffective. So there is no question as when you washed your penis or when you take in your paint etc.
