Is HIV Transmission Possible Through Oral Sex?

Question: Im 34 m, married. This is the first time in my life I have made a mistake outside my marriage. I had all my medical check done in my office in KL August end, my HIV and VDRL is negative.
I had unprotected oral sex (BJ) from a CSW in Kuala lumpur 7 days back. She initially took me into a room , washed me up with water and herself separately and gave me a clean towel to dry. I washed my mouth with water and she did the same. I was made to lie on the bed and she did some massage, i just kissed her on cheeks and neck and sucked her nipples slowly for 2 mins (No secretions came out). Apart from this I did not have sexual intercourse with her either vaginal or anal. Then she gave me a blowjob unprotected for 5 mins. I had french kissed her for 5 mins after the blow job. For briefly 30 seconds, I touched her clitoris with my right middle finger (Without cuts) and did not insert my finger in her vagina. Later she gave me a hand job and I ejaculated on my lower belly, later wiped it with a clean cloth. I wore my under vest and came back. I have not cleaned my penis with water after that. Can I get HIV from Oral blow job? I have not seen or smell of any blood in her mouth or vaginal fluid near me as the vagina was far away from my body.

Post this episode I have met following Doctors

1. Doctor in Brickfields (Saturday) provided me Antibiotic injection and 2 tabs (Doxicycline and Flagyl) for 5 days and said dont worry, you are getting anxious , do a test only for your satisfaction. Its very low to get HIV by this way.

2. Another Doctor in Brickfields (Sunday) again told me the same thing, if you want to get tested, do it after 6 weeks to relieve your anxiety. The transmission is low this way.

3. I visited an other STD specialist in Bangsar on Wednesday, he told me that if you had come to me within 72 hours, I had given you an anti viral pill. He examined my penis and took a urethral swab and said it was normal. He suggested me to get HIV tested after 4 weeks. He too said it is very very very low by this way.

4. I had counselling from 2 people including Mr XXXXXXX and said that it was negligible risk.

Please clarify if I may get HIV? I am very much annoyed and not sleeping.

If the CSW is HIV positive and she sucks my cock for 5 minutes, does the HIV transmit? I do not have cuts on my penis (Uncircumcised) . But I have not observed any blood in her mouth?? If it were to be, what could be the chances?

I have french kissed the girl for 2 mins after her blow job? Will this transmit?

I have not cleaned my penis after I wiped off all semen on my belly? Will this transmit?

Please tell me with your experience , any case like me with unprotected BJ leading to HIV?

I can also do the RNA HIV PCR testing if required?

Please tell me if I need HIV testing in this regard???

HIV is not transmitted through blowjob and kissing because human saliva does not transmit HIV virus and it also contains an enzyme which inhibits the growth of HIV virus so don't get feared or worried.

As such no testing is required but to allay your fear or anxiety you can go for 4th gen test after 4 week and the result will clear all your doubts. Be relaxed and move on as you are safe and not at risk of HIV.

One last query

I have not observed any blood in her mouth while kissing, if there were to be any while sucking even in small quantities . Can this transmit? This is what is worrying me

She also said that she does not have any disease and showed me the blood test mark on her elbow and she would get tested every 6 months.

And lastly, with your experience Doctor, did u come across a case similar to mine ending up in HIV? Thanks

Hi Doctor,

I met a HIV specialist and discussed my episode in KL.

He did a 4th GEN Elisa after 10 days and result came negative and all STI's were negative. He has 30 years of experience in managing this condition. He says you have no problems and move on. You do not need to test again.

Meanwhile I could not control my anxiety, I went to an other Doctor , I did not tell him about my ELISA result. The doctor said, I will test a Western blot on you to conform. My question is if the ELISA has come negative, What will happen to western blot? Its been a week , I have not got my results?

The experienced Doctor says, You are not at all at risk. I mean , if ELISA is negative, can western blot become positive? or indeterminate?

Hello Doctor,

The first Doctor told my ELISA is negative and conclusive. He told me even western blot is not required.

The second doctor does not know my ELISA results. He just did a western blot directly. I am awaiting results.

This happened because I did my Elisa test on September 10th and got my results on Sep 13 th.

I went to the second doctor on Sept 12th, I did not tell him about the first doctor, he straight away told Western blot.
I went to the second doctor and asked him that Can you rule out HIV in me? He said I will do a good test (Western blot) and he said surely it will be negative.

My basic question remains the same- The CSW has given BJ to me for 5 mins. I have french kissed her . Will BJ transmit HIV?

Why did the second doctor order me Western blot? I am really worried now if Western blot turns some indeterminate or something? Please help
What happens if Western Blot is done to a normal person?

My first Doctor told me that unless you are positive on Elisa, WB should not be done.

My first doctor says, I am not at risk at all. He says he can give in writing. But did I make a mistake by taking WB?

Both ELISA and Western Blot are good and reliable for diagnosis of HIV and it totally depends on your doctor which test he or she wants to confirm your status so don't get bothered or confused about it.

It is also true as said by your first doctor unless the ELISA is not positive there is no need for western test and it will obviously give negative result in the normal person so don't get worried or anxious.

My question is , I did Elisa 4th Generation on Sep 10th , results got back by Sep 13th , it was negative. I gave blood for Western Blot on Sep 12th; What could be the probable outcome? The two tests happened between a gap of 2 days only?
