Is HIV P24 Combo Test Done Post 84 Days Of Exposure Conclusive?

default
Posted on Mon, 4 Jul 2016
Question: Hello sir,
I took a HIV 1 and 2 antibody test (rapid visual test, method: immunochromatography) after 5 months of exposure to HIV at SRL diagnostics and the result came out negative. But at the time of taking the test the nurse used a Vacutainer and I did not see her open the seal in front of me. So I freaked out and out of tension I took another test HIV 1 & 2 P24 Combo (method CMIA) test at exactly 84 days at Dr. Lal Path Lab (this time he used a normal syringe and opened the plastic in front of me) after my first test. The sample had to be sent outside my state and I got the result next day.
My questions are:
1. Will the transportation of the sample effect the report at all. I have heard that HIV dies minutes after possible exposure to air.
2. Is the HIV 1/2 P24 combo test conclusive at exactly 84 days.
3. I took another test a different lab (HIV 1/2 spot test screening) at 85 days.
The result came out negative.
4. Do I need another test at 90 days.

Please help.

Brief Answer:
You can go for repeat test after 3 months.

Detailed Answer:
Dear XXXXXXX

Welcome to HCM.

The transportation of sample has no effect on the test result and it is also true that HIV virus can not survive in an open environment outside the human body and it will die within few seconds only.

HIV 1/2 and P24 Ag combo test is highly accurate and almost 100% conclusive at 84 days but as per routine HIV guidelines you need to get tested after 3 months to get 100% conclusive result because of the concept of window period related to HIV.

HIV 1/2 antibody test is also negative twice so you need not to worry at all because you are absolutely safe and free from HIV. Be relaxed and move on in life. If still any doubt left in your mind feel free to ask me any time. Always stay happy and healthy.

Thank You

Dear sir,
Thank you very much for your reply. I had a very unusual situation this morning. I feel down while going for a walk and got some scratches on my left elbow and there was bleeding. I immediately washed the wound with soap and water. My questions are.

1. Is there any chance of HIV infection provided that the area was urinated by some HIV infected person. I did not see any sign of it though.

2. If there is some stray syringe in that area and accidentally scratched me in the same area, will I be infected by HIV. I checked the area after 10 mins and found none.

Brief Answer:
No risk for HIV.

Detailed Answer:
Dear XXXXXXX

Welcome back and thanks for follow-up.

As I said earlier that survival of HIV virus outside of human body or in an open environment is very very difficult and it will die within few seconds only. The urine is also not the fluid which transmits HIV then there is no risk of getting the infection in both the exposures you had asked for. Calm down your self, forget about the incidence and move on in life.

Thank You

Sir,
I have one more worry. Once I had a tetanus vaccine at a private clinic. I doubt that the nurse might have cut her hand while opening the injection bottle. What if the blood from her hand got mixed in the injection solution that she injected me. Am I in risk? How early can a HIV 1 &2 p24 combo (CMIA) test detect HIV? Can HIV survive in TETANUS VACCINE solution? Please clear my doubts.

Thank you.

Brief Answer:
The way of transmission is very remote or practically not possible.

Detailed Answer:
Dear XXXXXXX

Welcome back once again.

The way of transmission of HIV mentioned by you is very unlikely or practically not possible because I have never seen such an exposure in my life.

HIV 1&2 and P24 combo test can detect HIV virus after 2 weeks of exposure or even earlier at 10days so don't get worried and move on in life happily.

Thank You

Sir,
Can the tetanus vaccine give false positive if I take HIV 1&2 P24 combo test after receiving tetanus vaccine. If yes then after how many days will I get the accurate result. Also assuming that blood got mixed in the tetanus vaccine. Will it be possible for HIV virus to survive and get me infected when I took the injection.
Please help.

Brief Answer:
It is not possible.

Detailed Answer:
Dear XXXXXXX

There is not any relation between the result of HIV 1&2 combo test with P24 Ag and taking vaccine for tetanus before it so don't be panic about that.

If blood got mixed with tetanus vaccine than also it is very unlikely to get infected because the route is very very unusual and no such case is reported till date so be relaxed and don't think much about HIV.
