Question: Hello Dr Varshney, I am following up on previous questions related to BPH. For three weeks I was only taking alfuzosin and herbal( saw palmetto etc). Today my Dht came 26 ng/dL ( reference range 16 to 79). Is this DHT level low enough to stop prostate growth or shrink it. Does it mean herbal stuff is good enough? Today with alfuzosin, I have started Avodart 0.5 mg - can I take avodart every other day to reduce side effects I had with proscar? Thanks. XXXX

Hello,
Welcome back.
Dihydrotestosterone (DHT) is the primary metabolite of testosterone in the prostate and hair follicle. Testosterone is converted to DHT by 5 alpha-reductase. So, 5 alpha reductase inhibitors help in reducing it's levels. So, the levels are low because of use of Proscar (Finasteride) that is a 5 alpha reductase inhibitor.
Avodart (dutasteride) is also a 5 alpha reductase inhibitor and will help in reducing the DHT levels, but the Alfuzosin has not great influence over DHT levels. Studies suggests that it also helps to some extent in reducing the DHT levels.
So, your DHT might be normal due to Proscar that you were using earlier or due to the herbal supplement.
If you are comfortable in taking Avodart and you are getting no side effects with it, yes, you should continue it along with Alfuzosin.

Hello Doc, Thanks for the answer. My DHT level of 26 - Is that considered a level where the prostate generally will not grow? What is the level considered necessary to shrink the prostate? Also is it ok to take Avodart every other day?

Sir, you need to understand that DHT levels are mostly indicator of the disease activity and the control. In fact most of the doctor's even don't perform this test as it has no sensitivity in regard to the control of prostate size.
There are no definitive levels that can define the prostate growth.
Avodart you may take on alternate day or as advised by your physician.

Last time proscar caused ED for me. I am concerned Avodart will do the same. Will Cialis counter this negative affect of Avodart? I once used 5 mg Cialis. It worked but I got headache about 18 hours after using it. What dose of Cialis should I use and is there any simple way to not get this headache. Thanks. XXXX

Hello.
Avodart and Proscar both belongs to the drugs of the same group so Avodart can also cause ED problem as well. So if you have to take Avodart then you may need treatment for ED as well. However it's not a rule as it varies from person to person.
Now as far as headache with Cialis is concerned, it is a prominent side effect of all the Phosphodiaterase 5 inhibitors. However it is seen minimal in peopt using Sildenafil at a dose of 25 mg a day before intercourse.
So in my opinion you should try using Sildenafil at 25 mg dose even if you don't get desired effect carry on with it for a dew days till you develop tolerance to this dose. Later you may increase the dose to 50 mg if the desired effect is not achieved.
Try it and let me know how you perform and feel after taking it.
