Is Cholecalciferol Advisable While On Chlorthalidone?

Question: Hi sir/mam..I am XXXX..age-26 years..I take chlorthalidone 12.5 mg and my bp is under control..i was thinking about starting with cholecalciferol 400IU daily as a preventive measure for osteoporosis..My question is will vit D3 cause over calcium build up in serum as calcium buildup is a side effect of thiazide diuretics? I was also thinking about starting magnnesium glycinate due to its high bioavailability at 100 mg dose as thiazide diuretics can cause Mg levels to go down..shall i start with Mg?

Chlorthalidone (diuretic) is commonly prescribed to treat mild to moderate hypertension.

Its most common side effects include hypokalemia (reduce serum potassium), hyperuricemia (increased blood uric acid), increased blood glucose, and increased calcium levels.

Taking Cholecalciferol together with Chlorthalidone can increase your blood calcium levels which may result in dizziness, weakness, lethargy, headache, nausea, vomiting or sometimes convulsions.

You may need a dose adjustment or special test if you use both medications.

Taken along with Chlorthalidone, magnesium glycinate may increase the risk of dehydration and electrolyte abnormalities that can lead to irregular heart rhythm, convulsion and kidney problems, hence it is not recommended routinely.

Beside medications you may need certain lifestyle changes to prevent osteoporosis like:

1. Exercise regularly and choose weight-bearing activities like walking and running.
2. Eat a healthy diet and lead a healthy lifestyle.
3. Avoid smoking as it has a harmful effect on reaching peak bone mass.
4. Have adequate exposure to sunlight as it helps in production of vitamin D in the skin.

Recently i was given the drug ctd-t 12.5/20 containing telmisartan 20mgn my bp is under control.....will a mild dose 12.5mg of chlorthalidone cause elevated sugar level?If yes,can i take any other diuretic in place of chlorthalidone which has less side effects with telmisartan?can i take any antioxidant to reduce elevated blood sugar if caused by chlorthalidone? can i take an emerging antioxidant coenzyme q10/ubiquinone present in mitochondria n tca cycle to reduce blood pressure..a lot of research has been done regarding co q 10 and hypertension..Thank you

Coenzyme Q10 has no role in lowering the BP.

Thanks for your query.
Owing to my busy schedule I could not reply it on time.

Coenzyme Q-10 or Ubiquinone has no BP lowering effect and as of date FDA has not approved it for any particular disease as such. It should not be substituted for prescribed medication which have a definite role in their efficacy and safety.

So, continue taking Ctd T (Chlorthalidone +Telmisartan) as prescribed by your doctor.

Another diuretic which is quite similar to Chlorthalidone is Hydrochlorthiazide which is also available in combination with Telmisartan.

Beside the medication, you need to have certain lifestyle changes such as dietary salt restriction, regular exercise, avoid smoking and alcohol (if you are consuming), eat home cooked food, increase the intake fruits and vegetables which provide sufficient amounts of antioxidants required for your body.
