Is CT Scan Good At Diagnosing Sinus Problems Particularly In The Sphenoid Area?

Question: Hi, both of my daughters have been diagnosed with New Daily Persistent Headache (NDPH) which they got after upper respiratory infections. Nothing they have been prescribed has been able to relieve the pain. I recently came across a few cases of NDPH actually being caused by sphenoid sinusitis. Would it be worth consulting an ENT specialist even though they have had MRI scans. I believe the CT scan is better at identifying sinus problems particularly in the sphenoid area?

I had gone through your question and understand your concerns that your daughters are having head-ache. I would like to ask you few more details.


Is there any nasal obstruction or post nasal drip (sinus secretions trickle back into throat) ?
Is their head-ache associated with vomiting or nausea or photophobia ?

1. NDPH (New Daily Persistent Headache ) or sinusitis can be diagnosed by detailed history, ent examinations and investigations.

Head-ache following respiratory tract infection, always occurs due to sinusitis.

2. CT scan of para nasal sinuses is a standard diagnostic modality for sinusitis (maxillary, ethmoidal, sphenoidal and frontal), where as, MRI reserved for to locate the complications (intra cranial) of sinusitis.


In my opinion, you need to consult an ent surgeon, to differentiate the cause. If necessary go ahead for the investigations.

Mean time, do simple steam inhalation, over the counter Tylenol (acetaminophen) half tab, twice daily (best works for head-ache).

Dear Dr Siddiqh,

They do have post-nasal drip but no nasal obstruction. There is no vomiting, nausea or photophobia. Would an MRI have definitely shown if there were an infection in the sphenoid area?
