Question: Im a 48 yr old male w anxiety high blood pressure and seem to avg low platlet count around 100 is my normal. Well over the the last week or so has noticed a stringy white substance left floating on top of the water when im done urinating and this morning it was actually Milky like at first but then seemed to cleared up I dont have any pain either

There are at time particles seen along with urine sample. Occasionally urine sample may appear cloudy. It doesn't carry major significance unless the incidents are isolated and not recurring. I would not be overly worried about the one time occurrence of white substance and initial milky urine as long as these instance don't repeat further. Should you notice many more instance of the same, you may consider getting urine sample tested under microscope (urine routine examination).

I can understand now. The repeated episodes of cloudy urine is worrying you. But many a times, the cloudiness is due to presence of cast/ sediments and sometimes due to infection. If you have not direct symptoms such as increased frequency / burning pain while urinating, abdominal pain then you need not be overly worried.

As I mentioned in my earlier reply sediments, casts in the urine sample besides infection can cause cloudy urine. At times, urinary tract infection and urinary sediments / cast may lead to kidney stones. However you are asymptomatic for kidney stones. Therefore I wasn't overly worried.

But if you are concerned and prefer to explore, you should consider urine routine examination. Based on the results of the test, you may also consider an ultrasound scan, a test to rule out kidney stones.

Both urinary tract infection and kidney stones are treatable condition. So stay relaxed. Drinking plenty of fluids helps a lot - both in clearing urine cloudiness as well as treating urinary infection / kidney stones.

Apologies for the late reply again. I'll answer your question in this way.

A kidney stone becomes significant when they are larger than 10mm. It's significant because those are the ones that might get blocked leading to pain. Stones smaller than 10mm more often than not pass away easily without disturbance. And stones that don't disturb a patient are often not diagnosed unless we do sequential monitoring through daily ultrasound scans. In other words, we run diagnostic scans in patients who complaint of symptoms and in already diagnosed patients. If I order an ultrasound scan in one with cloudly urine tomorrow, I may or may not see a stone. It doesn't necessarily mean this patient will not develop an insignificant kidney stone the next day or the day after

Therefore it's quite hard to answer your question. However if you have persistent cloudy urine, there is a definitive chance of infection. And a patient with urinary tract infection is more prone to get kidney stones. Hence it is necessary to do urine analysis and if necessary the ultrasound scan.
