Question: I have had ache in my shoulder for a couple of weeks. yesterday I woke up for work and it had spread into my upper arm. My thumb and index finger were numb. what could this be?

* Shoulder pain with tingling & numbness in thumb & fingers indicates pressure on nerve at back of shoulder joint due to various causes . It could be as simple as repeated up word movements of shoulder like in swimming, exercises or work involving the action or sleeping in awkward position - this leads to inflammation of Radial nerve & the pain / cervical spondylitis, cervical rib pressing over the nerve/ swelling of muscles, tendons or ligaments around shoulder / physical trauma to shoulder joint or inflammation of joint causing swelling & pressure on the nerve/
* The immediate home care involves - rest to the part, ice pack, over the counter medicines for pain( NSAID) like Ibuprofen pill taken after food.
Final diagnosis of cause is essential for full relief. .A detailed history of the pain, local examination to know limitations of shoulder movements & needful investigations to exclude diabetic peripheral neuropathy , X ray or MRI of shoulder, neck or cervical spine are indicated. Specific treatment would be anti inflammatory pain medicines, steroids( local or oral ), physio therapy ,
