Brief Answer:
Possible urinary track infection.

Detailed Answer:
Hello dear, welcome to Ask a doctor service.
I read your query and here is my advice.

Swollen ankles, shortness of breath, back pain and itching may all be signs of impaired kidney function.
Painful urination is not much related to that. It can be a sign of urinary track infection. You should do an urine analysis and uroculture . You may need to take antibiotics and can't neglect it taking count of your impaired kidney function.
Empirically I would treat it with ciprofloxacine.

Hope I have answered the question.
