Question: I am scheduled to see a colorectal surgeon in a few weeks to discuss removing a large ( maybe. 50. Centimeters). I am trying to get a pdf version of the report with images. I do not have this yet. I am sending by mail the report. ( doctor and pathology). I am also attaching an image of one page showing pictures ( poor quality).. my question is about removing a large sessile polyp at/( in .). Cecum.. after the surgery is it typical to recommend restriction of activities and how long.. For example, How long before I can lift a heavy dumbbell.. typically, will I have to change my food (eating). )

You have a large polyp in the cecum. As it cannot be removed during colonoscopy, You will should have surgical resection of small part of the large intestine (cecum) and the very end part of small intestine. Anastomosis between small intestine and ascending colon will be performed.
This surgery should be done laparosopically (keyhole surgery). The only reason to restrict activities is to prevent hernia formation in the area of small incisions. Regular activities should be avoided for 4 weeks. Heavy lifting should be avoided for 3 months.
Such small resection does not need to be followed by any diet change. You may notice loose stool few weeks afterthe surgery but it should subside by itself.

Thank you fir detailed answer. I see first surgeon in a few weeks. I sm so distressed. . How can a surgeon determine if polyp can be removed without destroying. Cecum. Prior to the laparoscopic procedure. Did you get to review the email. I sent? Any way to locate surgeons who have done this many times with minimal patient morbidity? I live in. Los angeles area, california.

A follow up clarification. Please. .. does the cecum. Have to be removed? ??????? the gastroenterologist said the polyp was a. 50 cm. Cecal.polyp. Is his considered very. Large?????.. I cant understand how there was no mention of a cecum polyp gollowing my colonoscopy and now. Its so large.

Yes I did gette review.
The polyp is quite large. Around 50mm which is 5 cm (NOT 50 cm). As technically it is imposible to remove just polyp, whole cecum ha to be removed. Especially that there has to be a mrgin of tissues not affected by it.
The polyp has been marked with an ink (during the colonoscopy), so it should be easily visible during the surgery.

I can not advise any specific surgeon, but You my find such surgeon typing "colorectal surgeon".

Thank you for your guidance, my meeting with surgeon is not until mid February. I will start tomorrow to see about other surgeons. Possibly get on a wait list. I would still ask if there is a sub specialty of co,ore talk surgeons that would have more experience with polyp removal surgery when the cecum must be removed. I would assume this is serious surgery and I would be willing to travel anywhere. Am I overly concerned because the cecum must be removed?.. ??…. Is there a way to find a surgeon that has done many, many of these?

Cecum can be connected with small intestine using stapler.

Hello.

Such surgery is not a hard one so there is no necessary to look for very specific cecum surgeon. Belive me, colorectal surgeon is good enough. This type of surgery is one of the most common intestinal surgery. We usually mechanically connect small intestine with large intestine using a tool called stapler. It stitches intestine walls with small stitches.

I am very relied and have reduced anxiety after your response. Here in s. Cal where I live, surgery for non malignant cecum polyp removal is 2-3 months away. How fast can. This change to malignant and should this be of concern. I am wondering if I should ask my internist to try and prioritize my surgery to avoid waiting? I thought of one additional question.. I am reading about EMD and EMR procedures to remove a cecal polyp. I would like to determine if. I am a candidate. For this procedure. Would you have any thoughts?

Your polyp is too large for ESD and EMR

Detailed Answer:
Generally it takes approximately 10 years for the cancer to develop from polyp. However this apply to small polyps. Your is quite large, so You can not wait that long. However 3 months is a safe period of time to wait for sure. This won't change anything and no need to prioritize Your surgery.
EMD and EMR are methods used for polyps not larger than 2 cm so Yours is too big. Moreover this methods are mostly used for stomach and esophagus lesions. It would be hard to do it through colonoscopy in cecum polyp which is that large.

My. GI. Doctor says the surgeon. May have to remove the entire. Ascending colon. This seems excessive. And. Very scary.. how does a surgeon determine how much colon needs to he removed. Very scary, st 78 years old, I wonder if. I should just leave the polyp and see what happens. Any advise is. Cery much appreciated.. thanks, XXXXXXX .. .

We remove whole cecum in case of a confirmed cancer. In other situations, it is not necessary to remove whole ascending colon. We may rather remove the small part of cecum.
Surgeon will see the ink in the intestine wall (visible as a black wall). And most likely he will focus on that part only. We must see if the blood supply will be sufficient to the other part of large intestine. We usually find the artery (called ileocolic artery) and close it (with clips). In the next step, wee must identify which part of the intestine remains with blood supply after artery closure. This makes a border between intestine which must be removed and one that can be left.
Moreover, if it will be whole ascending colon or just a part of it (cecum is a part of it), it should make no difference for You after the surgery. So the risk of whole ascending colon removal vs just cecum removal is simply similar and it is low.
I do strongly advice to do not avoid this surgery.

It’s difficult finding doctors and OR suits available. in the next few months. Who can advise me on waiting till March for any surgery and giving me time to make arrangements., The polyp has been marked with black ink.. the gasto dr. Says it’s. 50 cm. Sessile polyp. The picture does not show a 2 inch height. So. I am going to try and reach the doctor Monday. But I wonder if you have any thoughts on how the dr arrived at. 50 cm I. Size. It’s a mystery. To me. It’s very difficult to reach drs. Theses days. Medical professionals are over worked?

The polyp is 5 cm (50milimeters = 5 centimeters) not 50!!! If it were 50 cm You would be able to see it from the outside. Moreover You would have ileous symptoms.
