Question: I am 38 years old. I have Keratoconus in both eyes and have undergone cross-linking in both. Most recently I had "epi-on" cross-linking in my left eye, about 8 months ago. I am noticing continuing deterioration in my vision since that time. It feels like my vision is getting worse since then. At six month follow up they said it seemed slightly worse, but that that was normal, and that it often gets worse before it gets better. I am wondering if that is correct, and whether I should be asking for a refund or paying them to do it again. I am also wondering if there are any suitable contact lens options - I couldnt tolerate min-sclerals or hard contacts. I can tolerate soft contacts but they aren't strong enough for my right eye, which has very poor vision. Thank you

Brief Answer:
Repeat Orbiscan to check if redoing cross linking on cornea helps

Detailed Answer:
Hi,
I am a practising ophthalmologist for 10 yrs. I read through the details of your query.
Let me re-educate you that the procedure of cross linking done on cornea is not going to bring back your vision. This is a procedure to strengthen the cornea before steepening further. This would stop quick deterioration of vision. In keratoconus the lamellar fibres are degraded because of known reasons. The flatness of cornea is lost and gets steeper causing loss of vision and if not treated with cross linking it would lead to filling of the steep cone with fluid and cloudy (white) cornea. White cornea leads to blindness.

Now you should not ask for refund instead get an repeat Orbiscan to check for floats on the cornea. This scan will decide if repeat cross linking would help to stabilize the steepening cornea.

Hard contacts are the ideal and best to have which can stop the vision deterioration. You can try hybrid lenses which are rigid in the centre and soft to the periphery.
