Posted on Mon, 14 Jul 2014
Question: I am really insecure about the stretch marks on my breast. I hate them. I understand that there is no way to completely get rid of them, however I want to try anything that will make them less noticeable. Maybe fat injection or breast lift. My breast are slighty saggy and when I push them up I notice stretch marks are less visible. Please help.

Brief Answer:
Various modalities can be used to reduce the marks

Detailed Answer:
Hello young lady,

Welcome again to the forum and thanks for your follow up with important informations.

Before I proceed for the management of stretch marks let me guide you regarding your condition.

To tell you regarding stretch marks – Our skin is basically divided into three layers including superficial epidermis, middle dermis and deeper fat layer.
It is the dermal layer which is most important for the stretch marks. It is because of the fact that these stretch mark are formed due abnormality in dermis. Our dermis contains the special tissue known as collagen. This collagen acts in the same manner as air in the balloon. As soon as air in the balloon is expelled out then the balloon appears saggy and wrinkled. The same happens to dermis and collagen of the skin.

To explain you simply- whenever we gain weight then the skin stretches and therefore our collagen tissue also tries to expand in same manner. If we suddenly gain weight then the collagen tissue is not able to cope up with stretching of the skin and therefore the collagen at particular site of skin is also atrophied.

Till the time we have adequate of fat underlying the dermis, there is less visibility of this atrophy. As soon as there is reduction of underlying fat, there is more and more visibility of the stretch marks. This atrophy of collagen tissue is almost irreversible. But there is also an internal tendency of the skin to regenerate the collagen tissue which is expressed by bit redness and itching with mild elevation of the marks.

So you got some idea regarding your stretch marks. Now correlating with the information provided by you- your stretch marks appeared due to sudden increase and then decrease in weight. The mild disappearance as well itching is a sign of improvement in the stretch marks as self healing phenomenon. So it’s good news for you that your regeneration power is better.

Now for the part of treatment and other suggestions:

-     Let you control your weight part first which is the most important part in the management of stretch marks. There should neither be sudden increase as well as decrease in your weight. Keep a strict check on your weight profile.

-     Keep applying a moisturising lotion or cream two times a day. It will help in prevention of the stretching. The best time to apply a moisturising cream is just after bath when the water is still draping. So the additional amount of water mixed with moisturising cream or lotion will help you in hydrating your skin best. You can apply the cream for 3-4 times a day.

-     For the part of treatment- after analysis of the picture you can see that the stretch marks are lighter than surrounding skin which is comparatively darker. In your kind of cases there are two approaches which can be used:

1.     Using a cream which can make surrounding skin lighter. This approach will help you in masking the lighter stretch and merge with normal skin. For the same I use various creams containing Kojic acid, glycolic acid and azealic acid. It can be applied all over your darker areas up to neck region as well as on stretch marks. After a period of 8-12 weeks of application you will find visible results of merging of stretch marks with normal skin.

2.     The second approach is to elevate the atrophied stretch marks. This can be done by using Tretinoin cream over exactly stretch marks. Initially I use 0.025% cream and then gradually in 4 weeks interval I go up to concentration of 0.05 to 0.1% in next 8-12 weeks period. Initially it may cause bit redness and irritation but later on in few weeks it gets adjusted to the normal skin. A rule of thumb is that pea size amount is sufficient for palm size area, so that excess of cream should not be used.

3.     Additionally a cream containing Allantoin , cepae extract & sodium heparin can be used simultaneously.

Lasers are the latest modality but are under trail so I will not recommend you at this juncture.

I hope all these informations will help you. I will be glad to answer any further queries or if not then can close the discussion and rate the answer.

Brief Answer:
A regimen to be followed at day & night time

Detailed Answer:
Hello young lady,

Welcome to the forum again and thanks for your follow up.

As I guided earlier my treatment plan for your kind of problem is a kind of regimen explained in previous mail.

To guide you further- in my patients with your kind problem I guide them as below:

-     To apply a good moisturizing cream in the morning time and evening time after bath. It should be applied in plenty and liberally all over the chest and breast region. It is helpful in hydrating the skin.

-     Once a day cream containing Allantoin , cepae extract & sodium heparin (contractubex) can be applied in day time exactly over the stretch marks.

-     At night time initially on one day tretinoin can be applied and the other day cream containing azelaic acid, kojic acid and glycolic acid can applied. The tretinoin cream should be applied only on the stretch marks while azelaic acid, kojic acid and glycolic acid can be applied all over the chest and breast area including scar. The tretinoin cream helps in elevation of scars and the rest of creams helps in matching of the color.

Glycerin is not same as glycolic acid and is totally different compound. It would be best if you can take guidance of a dermatologist for application of all these creams and accordingly a proper follow up can be maintained.

My best wishes for healthy and glowing skin.

I hope all these informations will help you. I will be glad to answer any further queries or if not then can close the discussion and rate the answer.
