Question: Hi

What are options available for permanent full body skin lightening with even tone through out the body via plastic surgery ?
How much they will cost ?
Also mention their potential side effects if any?
Will they be permanent ?
What kind of post maintenance care they will require to manage the newly acquired skin tone ?
Please suggest me some place in XXXXXXX preferably XXXXXXX where I can get these treatments done?
Is it better to get these treatments done outside XXXXXXX

I have medium wheatish color and I XXXXXXX very quickly resulting in dark brown color.

I am Dr. Kakkar. I have gone through your concern and I have understood it.

It is not possible to change the skin type (the content of melanin which is genetically determined)of an individual. Your main concern seems to be tanning.

UV A part of the electromagnetic spectrum is mainly responsible for tanning.

Therefore, a good sunscreen should have broad spectrum UV A coverage in addition to UV B coverage, in order to be able to prevent tanning and it needs to be applied in sufficient quantity and every 2-3 hourly.

A sunscreen with minimum 30 SPF would be adequate. It should mention both UV B and UV A coverage. Therefore it is important to select the right sunscreen for desired effects

It should be applied in the following quantity:

--3ml for face and neck(a little more than 1/2 teaspoon)
--3ml for each arm(a little more than 1/2 teaspoon).

A sunscreen should be applied every 2-3 hours.

At night you can use a cream containing one or more of the Skin lightening agents e.g kojic acid, glycolic acid, arbutin or hydroquinone etc once daily to gradually fade away the XXXXXXX

Demelan cream is one such cream, MARKETED by gracewell/glenmark pharmaceuticals in XXXXXXX It contains glycolic acid, kojic acid and arbutin as main active ingredients.

You can also opt for a few peeling sessions from a dermatologist e.g Glycolic peel Or Retises CT peel (Yellow peel, also known as Skin lightening peel)

Oral glutathione capsules are also available online. Oral glutathione intake results in a lightening of skin color.

But i think surely there will be some such permanent treaments available.

Like skin melanin surgery which use selective ablation of melanocytes.

I have read that taking oral glutathione results in its poor assimilation as our stomach and intestine cant assimilate it properly,however taking its precursors like cysteine in form of L- Cysteine or NAC( more bioavlability than L -Cysteine may help )

Regarding Glutathione IV administration I am not sure.

What I was looking for some kind of plastic surgery which require minimal maintainace.Like using sunscreen 3 thrice a day along with some supplements and
some night creams.(as you suggested)

Reapplying sunscreen every 2 hours will be really hectic on a regular basis .

Irrespective of our skin color we all have same number of melanocytes,Only the melanin produced by them causes different skin color.

Supplement like glutathione, or its precursors,Vitamin C (3 to 4gm per day),Alpha lipolic acid may reduce its production by reacting between melanin synthesis pathways.

So is there anyway we can stimulate melanocytes to produce less melanin.
Or any other skin lightening alternative.

Please advice.

I am sorry but you have just encountered the most misleading articles ever. The desire to have white skin makes people believe that it is possible. These articles are so promising but not even close to truth:

and I am sorry but I don't agree with Dr. XXXXXXX Heijebu. I don't know what is he talking about in "http://doctor.healthcaremagic.com/premiumquestions/What-are-the-side-effects-of-skin-melanin-surgery/121317". I am appalled by Dr. XXXXXXX and to be XXXXXXX I am not aware of what is he talking about inspite of myself being a dermatologist. May be you can ask him a direct query and clear all your doubts. Laser may rather cause further darkening of skin in XXXXXXX skin because of Post-Inflammatory hyperpigmentation.

I ll give you a well known example of the great "Michel Jackson". He was black and became totally white (vitiligo like) later in life. Some say he was suffering from a skin disease called Vitiligo but actually what most think he did was chemical bleaching in which the skin loses all its melanin to become chalky white and becomes prone to sunburn and all that. The product he used was monobenzyl ether of hydroquinone, which irreversibly destroys melanocytes, mind you "Irreversibly" and it is not possible to control the degree to which you destroy your melanocytes thus instead of lightening the skin colour it may totally de-pigment it and sometimes in patches and that would look worse than ever.

Regarding Glutathione, your knowledge is correct. A low dose e.g 500mg, may not work as expected since most of the glutathione is metabolised before absorption. Therefore it is recommended to take a dose of glutathione i.e 1000 - 2000 mg per day in 2 divided doses to produce desirable results. IV glutathione is not an option and mind you, it is not for permanent fairness!!

I would still stick to my earlier advice and you may apply a broad spectrum sunscreen every 3-4 hours and practice other general measures like avoiding outdoor activities between 10 Am and 3 Pm, wearing full sleaved clothes, using broad brimmed hats and using Umbrella etc

So could you suggest me some good doctor in NCR region (Noida XXXXXXX XXXXXXX or XXXXXXX ) who can advice me about general skin care and daily skin regimen for keeping my skin healthy,young,rejuvenated and acne free in my natural skin tone.

To be specific i will be needing advice on

Daily Skin care or (Advice on my daily skin care regimen)
Hyper pigmentation on face and lips
Permanent hair removal (because I sweat a lot)
Tinea cruris cure (and regaining my natural skin tone of those regions)

So please advice accordingly.

Hi.

Skin care regime should be according to the skin type. Males have an oily skin. Moreover, acne prone is usually oily skin. A salicylic acid based face wash (e.g Spongysal foamy face wash) is ideal for oily and acne prone skin because it is exfoliating, comedolytic and unclogs the pores.

Besides, for acne you may use topical clindamycin 1% solution/gel during day time and Adapalene gel/Adaferin gel during night. Since, acne is a recurrent condition and there is no short term solution to it, therefore, once acne activity is under control, Adapalene gel can be used for maintenance of improvement because of it is comedolytic and antiinflammatory, thereby prevents new acne formation.

Hyperpigmentation, if it is post acne, can be improved with skin lightening creams like kojic acid. Apart from kojic acid cream, sessions of chemical peeling with either glycolic acid or Retises CT peel are also helpful. Sunscreens would benefit by preventing further sun induced darkening. However, it should be gel based preferably, so that it does'nt induce new acne formation.

Permanent hair removal is best done with a Diode Laser.

Tinea cruris is recurrent, specially if recurrent can leave behind post-inflammatory hyperpigmentation (PIH) along with lichenification of skin. In recurrent fungal infections due to constant scratching an element of eczema sets in and the skin becomes lichenified (dark and thickened) which itself is an itchy condition and thus it becomes a cycle, the more you scratch the more it itches and the more is the desire to scratch (Itch Scratch Itch Cycle). This cycle is perpetual if the fungal infection is recurrent.

The best way to deal with a situation like this is to treat existing fungal infection and prevent recurrence. You may use a mild-moderately potent topical steroid preferably in combination with an antifungal (e.g Clotrin B cream) twice daily, the antifungal would take care of the fungus and the topical steroid would take care of the lichenified skin. Along with this I would suggest weekly oral fluconazole 150 mg for 4-6 weeks.

Regular use of an antifungal dusting powder in the groin folds even after the condition abates is advisable to prevent the recurrence of fungal infection because it not only keeps the folds dry but also being antifungal exerts inhibitory affect on fungus growth.

An OTC antihistamine e.g cetrizine 10 mg once daily would provide you symptomatic relief from itching.

Laser hair reduction (LHR) is a very safe and reliable way for hair reduction. Apart from mild redness after the sitting which usually resolves in 1-2 hours, there are really no side effects at all, if done by a well trained/experienced expert.

Normally 6-8 sittings will give you a good hair reduction. It is permanent. It is US FDA approved for hair reduction.

The lasers commonly used are Diode Laser and Nd:YAG Laser.

Cost varies from site to site and is more if the treated area is more. For a full body LHR, it would be anywhere between 70k to 100k.
