Brief Answer:
Increase thyroxine dose, lifestyle modifications, medications

Detailed Answer:
Hi XXXXXXX

Welcome to HCM. I had gone through your question and understand your concerns.

Please share with us some more details like duration of thyroid illness, your weight height, if you have other illness like diabetes.

I have gone through your reports. My comments are:

1. You are having Hashimoto's hypothyroidism (antibodies high)

2. Thyroid function is not controlled. Your TSH is on higher side. On thyroxine the TSH has to be less than 3 miu/ml.

3. Your are having dyslipidemia. Cholesterol and LDL is high

My suggestions:

1. Thyroid:

It is desirable to increase the thyroxine dose to 150 mcg if you were regular with Thyronorm 125 mcg for more than 2 months. Once on thyronorm the target TSH should be between 0.5 and 3 miu/ml

2. Weight reduction

Your options of weight reduction are

a. Increase thyroxine dose and control hypothyroidism

b. Diet and exercise : frequent small feeds - low in calorie and high in fiber, regular aerobic exercise of 30 minutes a day for at least 6 days a week.

c. Medications - like orlistat, locaserin, phentermine/topiramate which are approved for weight reduction.

d. Surgical procedures - Usually we suggest surgery for patients who cant meet target weight in 6 months with above measures. Simple procedures like gastric banding or complex like gastric bypass can make desirable weight loss.

Hope I have answered your question. If you have any further questions I will be happy to help.
