Question: need guidance to maintain good health and also to reduce the glucose level which indicates i have diabetes.

Noted your concern.

Yes your blood sugar levels denote diabetes.
Usually diagnosis is based on Oral Glucose Tolerance Test (OGTT) or HbA1c results.

But in clinical practice Fasting & 2 hours post-prandial blood sugar levels used for diagnosis.

Any way your Fasting blood sugar level of 130 (Fasting blood sugar level is also a part of OGTT) denotes diabetes.

Also you are on right medicines for diabetes control (considering Type 2 Diabetes).

Important aspects of diabetes management are -
- Diet control :
One should consult near by dietician who can plan diet for him considering Body Mass Index & food habits.

- Exercise :
Usually 150 min per week of moderate degree physical exercise is recommended.

- Weight control.

- Regular medication and follow up with diabetologist.

- Regular screening for diabetic complications.
Fundoscopy to know retinopathy.
Diabetic foot examination.
Regular blood tests
HbAq1c
Lipid Profile
Renal function test.
Urine albumin creatinine ratio.

It is difficult to answer this question right now.

Usually once diagnosed, Type 2 diabetes requires treatment for almost life long.

You are on safe medications rarely have any major side effects. Can be continued for life long almost without much side effect.

What is important is –
To control blood sugar levels (Fasting, post-prandial & HbA1c) as normal as possible with regular exercise, diet control and medications with regular follow up with your diabetologist.

There is always possibility of reduction in number of medications or their doses depending upon blood sugar level.

Instead of thinking much about future, try to concentrate on short term goal.
Only time (future course of diabetes) can answer your question.
