Question: Yesterday i had sex with my girl friend and my condom was broken and my sperm got inside. So how to use mifeprin kit ? There is any side effects? Is it safe?

Answered by Dr. Nilaj Kumar Bagde (5 minutes later)
Hi, There is no need to use mifeprin. You need emergency contraception, Mifeprin is an abortificient. Use plan B one step or I pill. Just ask her to take one pill within 72 hours of sex as early as possible. This will prevent pregnancy and there is no need of abortion. It may cause nausea and irregular periods, so you may see your gynecologist for further details before taking the drug. Dr Nilaj

Brief Answer: Other options are there. Detailed Answer: Hello and welcome, I completely understand the issue. The other option is to take at least 100 microgram of ethinyl estradiol and a progesterone. Traditional birth control pills contain ethinyl estradiol or an estrogen along with a progesterone. One table has approximately 20 -30 microgram of estradiol depending upon the brand. So give her 4 tablets [ if they contain 30 microgram each] or 5 tablets [ if they contain 20 microgram each of ethinyl estradiol] of birth control pills. The dose is to be repeated after 12 hours. Also please remember to take the active pills as some packs contain active pills and 4-7 dummy or sugar pills. The sugar pills will not work. So she will need 2 doses. The first dose must be taken within 72 hours of unprotected sex and the second dose is taken 12 hours after the first dose. However this regimen is associated with side effects like nausea and vomiting. So please ask her to take this after meals and if possible give her a drug to control nausea like domperidone or ondansetron. If there is vomiting within 2 hours of taking the tablets then efficacy is not certain. The pills may also cause irregular period or delayed period. The pills cannot be used if there is active liver or kidney disease or heart disease. If this is not available then other options are getting a intrauterine device like copper T inserted within 5 days or use of low dose mifepristone within 72 hours [ not approved]. You will need to see a doctor for both. Also please remember that the chance of pregnancy after single unprotected sexual episode is not very high unless the contact occurs in the fertile period. The fertile period usually ranges from day 10-20 of the cycle in women with regular cycles. Even in these maximum chances are on day 12-16. So please do not worry. Let her take the birth control pills and they will protect against pregnancy in more than 97-99%% cases if taken soon. I hope that this was helpful. Please try to use ongoing contraception in future if possible. Feel free to ask any further queries and I will be happy to help. Thanks for using HCM. Have a good day. Thank your reply, one morething now she is zinoximor for utress infection. That is also safe?
doctor
Answered by Dr. Nilaj Kumar Bagde (7 minutes later)
Brief Answer: It is safe in most patients. Detailed Answer: Hi Zahid, Zinoximor is a good drug for infections. But it is an antibiotic. These are drugs used to kill bacteria. Not all drugs act against every bacteria. Some kill one bacteria but will not be effective against the other and so on. So we recommend to use them after testing that the particular bacteria is being killed by this specific drug and then give it. However some antibiotics are used empirically when we cannot wait for results of tests and it is an emergency. So in this cases this drug may be used. These drugs can cause serious reactions in a very small group of patients. So if there is shortness of breath, rash, giddiness after using this drug see a doctor. In most people it is safe and effective. For uterine infection I recommend to combine it with another drug called metronidazole thrice daily for 5 days. However if she is using the 4 birth control pills then use these drugs at least 24 -48 hours later as it may lead to severe nausea and vomiting. Hope this was helpful, Take care, and feel free for any further queries. Dr Nilajkumar.

Here available only XXXXXXX This contains Ethinlestradiol 0.030mg and Drospirenone 3.000mg . This not along with progesterone. Can i use this? And how many tablet have to take? Plz help me. I attached the snap of tablet box
doctor
Answered by Dr. Nilaj Kumar Bagde (9 minutes later)
Brief Answer: Ask for levonorgestrel containing tab if available Detailed Answer: hello and welcome, XXXXXXX does not contain levonorgestrel [ progesterone]. Ask the pharmacist for a pill containing levonorgestrel as the progesterone instead of drospirenone. If they are available and contain 0.03 estradiol take 4 tablets if contains 0.02 estradiol take 5 tablets and repeat after 12 hours. If it is not available take 4 tablets of XXXXXXX as there is no other option and repeat 4 tablets after 12 hours. Take as soon as possible as it is already almost 3 days. I understand the problems. I am feeling sorry that I am not able to provide another option. Take XXXXXXX if nothing is available as it will protect to some extent. If periods are missed do a urine test 3 weeks after the date of unprotected sex and if positive then can take mifeprin. Regards Dr Nilajkumar

Gestone ok?
doctor
Answered by Dr. Nilaj Kumar Bagde (13 minutes later)
Brief Answer: No not gestone, check list provided or take XXXXXXX Detailed Answer: Hi, No gestone is not OK at all. Gestone is not a birth control pill. It is a progesterone preparation different from birth control pills. It has progesterone other than levonorgestrel. Gestone will help in pregnancy so do not take it at all. Please understand that the progesterone has to be levonorgestrel. If it is not there then all pills will have effect but efficiency will be in doubt. Take XXXXXXX if it is not available. I am providing a list of pills that have levonorgestrel Altavera, Amethia, Amethia Lo, Amethyst, Aviane, Camrese, Camrese Lo, Daysee, Enpresse, Introvale, Jolessa, Kurvelo, Lessina, Levlite, Levora, LoSeasonique, Lutera, Lybrel, Marlissa, Myzilra, Nordette, Orsythia, Portia, Quartette, Quasense, Seasonale, Seasonique, Sronyx, Triphasil, Trivora or at least norgestrel. Cryselle, Elinest, Lo/Ovral-28, Low-Ogestrel, Ogestrel I understand that there are problems at your end. Ask the pharmacist for a pill containing levonorgestrel or choose from the above options. If not then XXXXXXX is the only option. Take care. Dr Nilajkumar.

She already taken XXXXXXX Day after tommor i wil get i phill from india. But day time wil be morethan 72 hours. So there is any use of that??
doctor
Answered by Dr. Nilaj Kumar Bagde (16 minutes later)
Brief Answer: It can be used till 120 hrs but less effective Detailed Answer: Hello and welcome back, I pill can be taken upto 120 hours after sexual episode, single dose of 1.5mg. However the protective efficacy is less [89%.]. But once she has taken XXXXXXX it is not advisable to take I pill. Ask her to take the second dose of XXXXXXX after 12 hours. Any one of these regimens can be used only once in one cycle. So even if there is another sexual episode after this and you plan to use emergency contraception, it is not advisable and we do not know if it will even work the second time. So if XXXXXXX has been taken, there is no need for anything else. Just wait. The chance of pregnancy after a single sexual episode is less and XXXXXXX will also protect. So there is no need to worry so much. Get the I pill though and keep it for future use. Dear Zahid,I understand your concern as abortions are not legal there. But there is no need to worry so much. You have done everything that was possible. So just wait till she has a period. It may be irregular due to the pills. So do not worry if it is missed for a few days. If period is missed for more than 7 days then get tested for pregnancy. Hope this helped. Dr Nilajkumar.

Answered by Dr. Nilaj Kumar Bagde (8 minutes later)
Brief Answer: This is a known but temporary side effect. Detailed Answer: Hi Zahid, I had already clarified that this may cause nausea and vomiting. I had also suggested to take tablet ondensetrone with these to help decrease the nausea. Please get it and give it to her. This is temporary and will go away after some time. Do not get scared, Just let her take small sips of juice, or soup or water after every 10-15 minutes. There will not be any problems in future. This regimen is difficult to tolerate but you did not have any other drugs. Just have patience. If she continues to have nausea and vomiting even after 3-4 hours after taking tablet ondensetrone see a local doctor. I understand your situation. Take care. Dr Nilajkumar.

How many hours i have to wait before see a doctor?
doctor
Answered by Dr. Nilaj Kumar Bagde (18 minutes later)
Brief Answer: It depends on her condition. Detailed Answer: Hi, If she is having too much trouble, you may even go now. Do not wait much if she is in distress as these severe symptoms are unusual. Women do have vomitings but they occur for 3-4 hours maximum for 6 hours and then mostly they decrease or recover. If she is comfortable and vomitings are controlled and is able to take sips of water or liquids or cold milk, there is no shortness of breath or chest pain, she is responding well to your talk, she may not need to go. Dr Nilajkumar.
