Question: How long should I continue my combination of metaprolol 25mg, ecosprin 150mg, prasugrel 10mg and atorvastatin 10mg? I had an MI, PTCA stent to LAD in 2010 when I was 38 years old. I have normal cholesterol levels, BP 110/70, 5'10" 63KG. I have been taking these medicines for the last 4.5 years.

Dear Sir
The requirement of these medications depends upon a number of factors which include clinical examination also, however, I will detail general principles in following paragraphs.
1. Ecosprin and atorvastatin should be continued life long in any individual who had an MI, unless there are problems associated with its use or there are contraindications. These are prescribed to prevent future heart attacks and stroke. However the dose of atorvastatin may change from time to time.
2. Metoprolol is another essential drugs for those who had an MI. However dose will be regulated by the treating physician.
3. Prasugrel like drugs are usually stopped after 1 year of stenting. Use beyond 1 year should always be for certain valid reasons and with consent of your treating physician. Since you had an MI at a young age, the treating physician may like to continue it for longer period, however , 4.5 years is a long time. You must discuss regarding this drug with your treating physician who performed the PTCA, as the choice of stent, type of lesion etc are important considerations. In my practice I do not use prasugrel after 1 year of stenting unless there are reason to believe that patient has high thrombotic milieu or has active disease.
4. I rarely use fenofibrate in my practice.
