Posted on Tue, 15 Nov 2016
Question: i got "VDRL - reactive 1:4" and "TPHA-Negative". I am taking medicine to get VDRL as Non reactive for visa purpose. Skin specialist suggested me to take Azythromycin tables for 4 days though they confirmed that i am sypilis negative. So, after taking the tables, after how many days i can go for VDRL test again?

Brief Answer:
The cause for Biological false positive has to be identified.

Detailed Answer:
Hi,
Welcome to HCM.
Thanks for posting your query.
VDRL reactive, but TPHA nonreactive means you are not having Syphilis. It is called as Biological False Positive. BFP will be less than 8 dilutions. It can occur in the presence of diseases other than Syphilis.
Two types of BFP:
1 Acute BFP-- Positive less than 6 months
2. Chronic BFP--- Positive more than 6 months (sometimes for years).
Try to identify the underlying diseases (with investigations) and treating the same will help you to get a VDRL nonreactive.
Azithromycin 4 days can not be the solution for BFP. Consult another experienced physician or who is an expert in STD management.

Brief Answer:
No treatment is necessary at present

Detailed Answer:
Hi,
Welcome back.
Once again check TPHA test in some other reputed laboratory. If that also nonreactive, you need no treatment for Syphilis. In that case, try to fix the cause for biological false positive result with investigations. Your physician will guide you for the same. If no cause is found, ignore the RPR reactive report. Repeat the test after 6 months. Treatment has to be considered as per the outcome of the investigation reports. Consult your physician or Dermato-venereologist.
