Posted on Wed, 9 Dec 2015
Question: Was on 20mg Lisinopril/25mg HCTZ but developed Gout. PCP DC'd HCTZ & Rx 40mg Lisinopril. Didn't work so added 12.5mg Coreg bid w/the 40mg Lisinopril. 9 days later BP= 164/90 standing (manual) & 171/101 (sitting) taken on Lifepack 15 monitor. Dr switched me to Losartin 50mg bid 3 days ago, & BP was 201/101 sitting w/Lifepack15 24 hrs ago & 158/80 manual sitting today. I'm 64 Y/O W/M & drink 16oz black coffee before each BP check. Latest lab= normal HDL & LDL but A1C at 6.0 ( down from 6.2 taken 30 days prior). QUESTION: Am I just not waiting long enough for Losartin therapy to work?

Brief Answer:
should be working after 3 days

Detailed Answer:
HI, thanks for using healthcare magic

It is possible that the coffee is playing a role, it should not be taken within 30 minutes of a reading because it can affect blood pressure values.
This is part of the guidelines in bp measurement.

If you using the coffee 30 mins or more before BP measurement, then it may not be having an impact.

After 3 days, the medication should be exerting its effect so if your blood pressure is not reaching goal at this time then a small dose of additional medication may need to be added.

According to JNC 8 US blood pressure guidelines, in persons older than 60, who do not have a history of heart disease, the goal is less than 150/90.
If there is a history heart disease then it has to be less- 140/90 or less.

Diabetes- 130/80 or less

Summary: (1) after 3 days, medication should be exerting its effect, may need additional medication
(2)if using coffee within 30 mins of reading, need to use it before that time or , even better, after taking reading

I hope this helps, feel free to ask any other questions
