How Is Herpes Transmitted?

Question: Hi
I had a sexual encounter with a masseur in Bangkok last week. I kissed her for 20 odd seconds, then she rubbed her body against mine, gave me a blowjob by putting on a condom on my penis (did not cum), then she removed the condom and made me cum by rubbing my penis with her hand.

I'm worried that I might have encountered any STD particularly herpes. I have a couple of normal sores on my body now. One behind my shoulder and the other very small one on my thigh. The puzzling thing is that before I had this encounter, 2-3 weeks back, I had some small sores on my thighs and buttocks. I'm not sure whether I was exposed to herpes previously or I'm now. The last sexual encounter I had was 3.5 years ago. Shall I go and get tested. Or I'm thinking too much. :(

Let me tell you certain important facts related to herpes:

To tell you medically - Herpes lesions manifest itself as multiple closely placed small vesicles (fluid filled blisters) which remains for just a day or two and then forms ulcers which are quite painful. These ulcers then heals in next 5-7 days.

To begin with also these vesicles are quite painful and itchy with discomfort and once the vesicles burst in a day or two then the ulcers which are formed are quite painful for next 4-5 days. So the symptoms can never go unnoticed.
In yourself I can make out that no such symptom is present matching with the above symptoms.

So you can rest assured regarding the rarest possibility of herpes. You can make out that you do not have any of the symptoms which match with herpes or its symptoms or any other Sexually Transmitted Diseases.

In my clinical experience as well as academically speaking kissing for 20 seconds or rubbing or hand job does not cause any such type of lesions which you are telling.

Thanks Dr XXXXXXX for your reply. You did help me calm my nerves but I'm experiencing a lot of anxiety these days. I do not have a very high resolution camera right now but I'm attaching whatever pics I could click with my smartphone. This sore is on the back of my shoulder. This sore developed yesterday and it is painful right now.

The main tension that I'm going through is I'm worried about passing this disease to my girlfriend through kissing. I'm not sexually active with her apart from kissing.

The doctor\counselor that I visited asked me to get tested for HSV1/2, HBsAg and VDRL, Ig after 4 weeks from now. But since I'm feeling a lot of anxiety can I get tested once right now and then get tested once again after 4 weeks. Doc, another question before you reply. I have noticed another couple of symptoms which might be just normal things. Today afternoon when I pooped, I noticed irritation at the point from where I poop. This happened last week too but went off the next day. I'm not sure what is causing this.

Also, I have noticed some pain in my testicles which happens at one second and goes off completely then. It's not persistent and goes on and off sometimes. I have also noted a decrease in appetite.

Can these things be considered as symptoms of any STDs? or it is just normal and I'm overthinking about everything as a symptom. I'm worried about these things so much that I stay depressed all the time and think about living my entire life alone if I get any STD. :(

Just in case you miss this question in my second follow up question I'm pasting it again:

My dear young buddy,

Welcome again and thanks for posting your pictures.

After reading your query with diligence and correlating with pictures it seems that you have the problem of "Recurrent Folliculitis". Mind it that the condition is not at all herpes.

To tell you simply about this Recurrent Folliculitis - This is a condition of inflammation and infection of the hair root and oil gland attached to it.

To explain it medically and scientifically in simple words- we have hair all over our body which have opening of the oil glands attached to it. By many a factors these hair root and oil gland can become inflamed and infected leading to folliculitis.

The reason for the inflammation and infection of the area is due to the fact that our body harbor multiple types of bacteria on the skin which are known as “Normal flora” of the skin. A bacteria “Staphylococcus aureus” is also a part of that flora, but as soon as there is some break in the skin or trivially also due to bit ups and downs in immunity, the bacteria gets access into the skin and causes the aforesaid process of inflammation and infection.

When it is superficial it heals without any problem, but when it is deep it causes lot of pain and swelling around the area. In general these persist for few weeks and heal without any problem. After a period of time our immunity prevents the further formation and inflammation.

There are certain important tips regarding your problem:

1. Do not pinch, pop or even try to touch any of the area.

2. Kindly avoid scratching any area of normal skin also, as it leads to minor entry points for the entry of bacteria.

3. Wear cotton clothes and cotton undergarments to be changed two times a day.

4. Maintain proper hygiene and take two times daily bath.

5. Avoid any kind of stress, even for this condition also as stress is one of the very important factor playing role in our immunity (making it low) so prone for infections. Yoga, meditations and simple walk do help a lot in improving immunity.

6. For the present condition now, you can do warm saline compresses (1 glass of water and pinch of salt in it) for 2-3 times a day.

If I were your treating dermatologist then would have suggested you to apply plain antibacterial cream such as fusidic acid or mupirocin (latest is retapamulin) and to take pain killer tablet such as Ibuprofen or paracetamol by mouth for reducing the intensity of pain and redness.

If these are enlarging and painful, then along with above mentioned measures you would require a course of oral antibiotics (best after culture and sensitivity of pus) under medical advice. If at all you take oral antibiotic, then do take proper dose and full course of antibiotics (as improper dosing and schedule leads to antibiotic resistance and then recurrent nature of the situation).

Eat healthy and nutritious food to improve your immunity at its best because is the most important factor in reducing and eliminating any infection from the body.

Now we come on to your point of testing- In your kind of exposure there is no chance that you would get any type of STD. I counsel so many of your kind of cases and have never found any kind of STD in other persons with your kind of exposure.

So first and foremost relax yourself and do not worry. So, as per my experience I assure you fully that you have not contracted any sexually transmitted infection. Therefore at any time if you go for any testing your tests would be negative because of your clear status.

Thanks a lot Dr. XXXXXXX for the encouraging words. I am feeling better after reading your message. Thanks again. I did however went to a lab today and gave my blood for testing of the STDs mentioned earlier. The report will be given to me on Wednesday evening. I am hoping everything comes out negative. A couple of questions for you again. Sorry for any repetition.

1. Is there anything I can do for testicles pain? It is coming and going today quite frequently. I'm wearing a loose undergarment but still I'm feeling recurrent sensations.

2. Also, the medical condition explained by you, is it the same cause for irritation while pooping? I was feeling the irritation while sitting too but it's little better now.

Since all these things are happening at the same time, it is adding on to my anxiety. Since this is the last free follow up question, I will reply again with the result of my reports on Wednesday.

First and foremost just relax yourself. You are totally clean and there is nothing to be worried about.

Your testicular pain and anal pain are not related to any sexually transmitted infection. It is just coincidental that you are getting all the small problems simultaneously after the incidence.

In my opinion it is just spasmodic pain which can occur quite usually. Drink lot of water and do not sit for long hours.

for pain during pooping- avoid any spicy food and reduce your stress level.

I am fully confident that your test results would be cent percent negative. Do give me a follow up of negative test reports!!!

I got my test results. Although everything looks negative, I'm kinda confused by them. Following is what is written:

I have some questions:

1. Although the test results fall in the negative range, why am I getting some values in the OD Ratio? Shouldn't it be zero if I don't have any antibodies for the HSV virus?

2. Is high HERPES II IGM indicator of anything?

3. Also, at what time shall I test myself again to eliminate the risk of any STDs completely. Since this time I have tested hardly 8 days after the exposure, if I test now after 5 weeks after the exposure, will that eliminate the risk completely?

I am really pleased to know that your reports are normal and negative. As i said previously that in my clinical experience all your reports would be negative. The same thing applies to all the tests if done at 5 weeks period.

For declaring you completely negative there is no time better than this. But even if you academically ask then there are different of the time periods for each test. I suggest you to calm down you nerves and relax yourself. In a roughly estimated time we consider 5 to 6 weeks of time for testing most of the issues.

I again firmly declare that in your type of exposure it would be cent percent negative even at 5 weeks testing.

Now for your further query of OD values in herpes- An OD value is an optical density of the test by which we declare the test positive or negative. The serum and the test contents illuminate a light according to its presence or absence of antibodies against particular virus. Even if it is negative then also there is some illumination if light by the test contents which is a normal natural scientific fact. It can not be negative because of the above fact. If the optical density is zero (which actually never happen) then the test is considered invalid.

So you got that it is quite obvious to find your kind values in normal negative test. So do not worry about these values and I assure you fully that the are totally correct and declare you negative.
