10 cc of MultiHance administered without complication
TECHNIQUE
Sagittal and axial T1-weighted images, axial gradient echo, axial FLAIR images, axial diffusion weighted sequences, axial T2-
weighted images of the brain were obtained. Following gadolinium administration sagittal, axial and and coronal T1-weighted
images were obtained.
FINDINGS
MRI of the brain demonstrates scattered focal white matter hyperintensities seen. The more discrete regions are noted with a small
linear focus of increased signal in the left frontal corona radiata (axial series 7 image 20) and a few focal subependymoma lesions
suggested overlying the left frontal horn and left periatrial region (axial series 7 image 18-16). A vague blush of increased signal is
seen overlying the right and left occipital horn. There is also a linear focus of increased signal seen in the subcortical white matter
of the anterior right temporal lobe (axial image 11). A small focus of increased signal is also seen overlying the right frontal horn
(image 16). These foci appear similar to the prior exam. No abnormal enhancement is seen.
No lesions are seen within the basal ganglia, brainstem or posterior fossa. No distinct hyperintensity seen within the corpus
callosum. Sparing of the U fibers is noted.
Such hyperintensities are nonspecific. These may be seen secondary to prior infectious/inflammatory processes, including
arthritides and connective tissue disorders, demyelinating disorders or chronic end vessel ischemia, including vasculitis. White
matter hyperintensities have also been described in patients with migraine headaches. Small hyperintensities have also been noted
in the normal population.

There is no acute infarct seen. No intracranial hemorrhage is recognized.
The ventricles, sulci and basal cisterns appear unremarkable.
There is no enhancing lesion and no significant leptomeningeal enhancement.
The vertebral and internal carotid arteries demonstrate expected flow voids indicating their patency.
The sella is unremarkable. The central skull base and temporal bones are intact. The calvarium appears unremarkable.
The orbits are unremarkable.
The paranasal sinuses demonstrate mucosal thickening partially outlining anterior and posterior ethmoid air cells with a small
hyperplastic polypoid focus in the floor of the right antrum. No air-fluid levels are noted. The mastoid air cells are unremarkable.
The nasal cavity appears intact. The nasopharynx is symmetric.
IMPRESSION
1. Scattered small nonspecific white matter hyperintensities, similar to the prior exam. No enhancement.
2. No cerebral ischemic changes seen. No acute infarct.
3. No parenchymal mass or hemorrhage.
4. Mild sinus inflammatory changes.

I am just looking for some clarification regarding the radiologist's report (pasted above).

When he writes "few focal subependymoma lesions suggested overlying the left frontal horn and left periatrial region" does he actually mean subependymal? I think there is a typo but wanted your opinion regarding the rest of the report.

I've gone through your query and understand your concern,

Subependymal region refers to area adjacent to lining of lateral ventricles in brain.

MRI findings are same as compared to previous findings.White matter hypo densities are non specific and it may be due to many causes like age related ischemia,sequel of inflammation,infection etc.

There are also signs of inflammation in sinuses.

In short,findings are not serious.
