Thank you so much for the updates.
I would now guide you regarding the process for medical abortion using the MTP Kit.
This kit contains one tablet of Mifepristone and 4 tablets of Misoprostol.
Generally one tablet is taken orally ( mifepristone ).
After 24 hours, 2 tablets of Misoprostol are taken orally or inserted vaginally.
After another 24 hours ( 48 hours after first tablet of Mifepristone ), 2 more remaining tablets of Misoprostol are taken again orally or vaginally.
If she vomits the dose within 4 hours of oral intake, it has to be repeated.
After taking the tablets ( any of them ), she would start bleeding within 24 - 48 hours.
Bleeding would be like a heavy period, with cramping , passage of blood clots or a fleshy gray mass.
The bleeding will be heavy for 3 - 4 days it might continue as lighter bleeding or spotting for 2 weeks.
Cramps are to be expected , and it is best not to take painkillers for this.
Mild nausea, fever, headache maybe expected as side effects.
That much is for the explanation of how to take the MTP kit.
NOw I have a few suggestions for you -
Medical abortion should NEVER be attempted without consulting a proper gynecologist.
The pregnancy diagnosis should be confirmed by an ultrasound scan.
There are 3 reasons for this.
1. The pregnancy has to be accurately dated in weeks and days, as the pills work within narrow time frame.
2. The pregnancy has to be confirmed as inside the uterus. A rare condition is ectopic pregnancy ( pregnancy outside the uterus ) , which can be life threatening if not detected.
3. She has 2 previous Cesarean sections, the pregnancy might be stuck to the earlier scars of the surgery, in which case pills will not work, and it is a tricky situation for even surgical abortion.
The pills also should be taken on prescription by a gynecologist, so that if there are any complications such as heavy unexpected bleeding, fainting, infection etc, then the doctor is already briefed and readily available.
It can be very dangerous to take these tablets on your own without guidance of a doctor.
After 2 weeks of taking the tablets, a follow up ultrasound scan should be done to confirm that the abortion was successful.
ALso, the pills are successful in 95 % of cases, but with prior surgeries on the uterus, the failure rate increases slightly , and she should be prepared for the possibility of undergoing a surgical abortion if the pills do not work.
If you carry out the procedure under expert gynecological care, then it is a relatively safe and easy way to accomplish an abortion.

She had MBT kit and after that she has got bleeding for next four days and later went to doctor after 10 days , after ultra sound dr told her pregnancy has been completed but some particles are still there so again she suggested that due to these particles infection is possiable so better to go for surgery .

The patient donot want to go for surgery and please suggest if there any antibiotic through which these can be treated and also suggest whether the pregnancy has been completely aborted or any chance of recurrence

Yes, as I told you, with prior surgeries, success rates of medical abortion decline .
Can you upload the ultrasound scan report ?
If the retained products are more than a certain dimension, then the doctor is absolutely right that it is better to go for surgery.
I need to look at the scan to see what is the next option.
If the products are not very large, then it is possible to have an option of repeat dosage of Misoprostol, that might sometimes remove the remaining products.
IF the repeat dosage also fails, then surgical abortion is a must to prevent infection.
Antibiotics are compulsory whether repeat medications are tried or surgery is done.
The best antibiotic can be suggested after looking at the scan report.
